Subject: Incident report: Sudden change of schedules

July 15 2020, I Rhodel Calma, nursing attendant in IFF had an emergency. I was unable to attend
my designated duty because I needed to attend on behalf of my grandmother, to attend the mandatory
reporting in Veterans office in Clark, Angeles, Pampanga, so she would not be remove from here yearly
pension benefits. It was a sudden notice from the Veteran’s office, Therefore I decided to ask Ma’am Joy
to tend my duty on my behalf for exchange of that I will take her duty at July 17, 2020. She agreed upon
it. Afterward, I Inform Mr. John Bulario on my behalf to inform the higher ups on my sudden actions.
With due respect, It was hastily decision of my mine of what I have done. I do humbly ask for forgiveness
for my errors. I will do reporting in proper in chain of command next time, if the same incident will
occur. I do appreciate of you taking time for this incident and I do hope that you would be just in giving
the proper action.
