Dear Ma’am,

I am BENIGNO D. PAULINO and a community nurse under DOH program for almost a
decade now. As a registered nurse, I am also a Covid Warrior since the pandemic
strike the country. I was augmented multiple times in different municipalities of Samar. I
was one of the few who volunteered in the National Capital Region during surged of
COVID-19 from April to May 2021.

I am writing this letter to ask for a reconsideration on my insurance claim. I met

an accident last August 10, 2021 while on my way home from my duty at Sta. Margarita
RHU which damaged my ____ knee. I was hospitalized twice (sometime in August
2021 and September 2021). And as a consequence of the accident, I underwent
surgical operation to fix my ____ knee.

I filed may accidental claim on _____ which was unfortunately denied allegedly due to
late submission of supporting documents. I was told and instructed by the DOH
personnel to write this letter and explained my reasons for late submissions of the said
documents pertaining to my accidental insurance claim. Here are the following reasons;
on the time of my vehicular accident our family was mourning for the loss of my
grandmother while i was hospitalized and suffering from pain both physically and
emotionally. It was a difficult time for us since I wasn't able to visit my family after I
came back from my volunteerism in NCR and be back in my duty in Sta.Margarita in the
second week of June two Months after my grandmother passed away. After my
operation I had a difficulty in doing my daily activities and needed assistance in my daily
life. Due to pandemic I’ve had a hard time processing my papers because my mother is
senior citizen and my youngest brother was sick due to dengue, a difficult time was into
it and I don’t have a father to be with us. I am living with my mother who is a senior
citizen, i stayed in a far flang abrangay of calbayof city to recover because I don’t have
house and relatives in the city who can take good care of me, there is no signal in my
barangay and communication is one of my problem. I had a difficulty traveling to the city
due to my limited movements because of the cast on my knee and the mode of
transportation is only center car which is a burden to my operated knee, and the road
is rocky that caused pain in my affected area every time i travelled for my follow-up
check-up.
Another reason that lengthen the process was my doctor, the only Ortho who is from
Tacloban and only have 2 days of duty in the hospital, his signature was needed since
he is my doctor. I was referred to therapist and until now I'm undergoing therapy and I
am hoping that you could consider my appeal, and my claims is a big help to sustain
my therapy which is costly and needed for my fast recovery.

I am asking that you reconsider my reasons to grant my accidental claim. Should you
