KNOW ALL MEN BY THESE PRESENTS:

This Addendum to the Extrajudicial Settlement of Estate made and entered into by and
between;

_____________________, widow, ___________________. Single ______________________________ married to

______________, with residence and postal address at Brgy. San Nicolas Poblacion, Concepcion, Tarlac,
all of legal ages, Filipinos ;

WITNESSETH

That the parties previously executed an Extrajudicial Settlement of Estate dated October 9,
2007 sworn to before Notary Public ______________________ appearing in his Notarial Register as Doc.
No. _______, Page No. _____, Book No. ___, Series of 2007.

That the parties agree to the following addendum to the Deed:

WHEREAS the abovementioned Extrajudicial Settlement of Estate does not indicate

the civil status of the heirs of Cecilia N. Valencia ;

WHEREAS the correct civil status of the heirs are the following:
1. Orfelina N. Valencia - Garcia - widow
2. Sr. Ma. Ligaya N. Valencia – single
3. Sylvia N. Valencia – Hipolito – married to Remigio Hipolito

That all other terms and conditions as contained in the aforementioned Extrajudicial
Settlement of Estate shall remain and continue to be binding and effective between the parties
insofar as they are not inconsistent with the provision of this Addendum .
