Respected Sir,

• I am studying a subject, Nursing Research in VIII semester of Generic BSc N, 4 Years degree
programme. This is the requirement of HEC curriculum to conduct a research in this subject on the
legal aspects of nursing. To fulfill the requirement of course and for better understanding, I want to
conduct a research study on a selected issue, which is legal bill of rights of terminally patients in hospital.
I need to collect opinions, suggestions and factual data from the health care professionals of CMH
Kharian.

• The results of this will add up in the knowledge of patients, nursing cadets, clinical nursing staff,
administrative staff, physicians and other readers in the future.

• Ethical consideration is being taken care by attaining your kind permission as institutional consent and
in the form of Individual consent, (consent form is attached as Annex-‘A’). Questionnaire is also
attached for your approval (Annex-‘B’).

Significance of study: The study is designed to conduct a research for the knowledge, attitudes and practices of
nurses in therapeutic abortions while remaining in legal brackets.

Procedure: You are requested to participate in this study. You will have to consume 15-20 min to fill the study
questionnaire.

Ethical considerations: Permission has been taken from commandant, CMH Kharian as an institutional
consent.

Possible risks & benefits: This study has no harm but will add up the knowledge to nursing cadets, clinical
nursing staff, administrative staff, physicians, readers and health care stake holders.

Right of refusal to participate or withdrawal: Your participation in this study is voluntary. You have right to
