Rural Internship Report week 4(2/02/ to 7/2)

Today was the immunisation day for kantheshwar subcentre. In PHC system, every first Monday of the
month will be fixed as the immunisation day for kantheshwar SC, every Tuesday for erandeshwar SC
and for all the 10 sub centres under our PHC, a first week day of the month will be given as fixed.

The people will be already acknowledged regarding this day and ASHA workers will be informing the
targeted families as the day is approaching. Around 78 children of different age groups were supposed to
come and receive their respective doses. The camp started around 9AM itself. The vaccines were taken
out of the deep freezer and kept in the cold boxes. There is marked vaccine stand as well to keep the
vaccine vials as soon as they are taken out of the cold box. There is a separate Vaccine vial monitor,
which is a safety mechanism to check the potency of the vaccine. The colour indicator for the VVM is
given on the labelling itself. The interpretation of the VVM and its importance has been described by
our MO. Along with the vaccine, a tablet of paracetamol was also supplied to the children, incase of any
allergic hyperthermia happens.

The immunisation camp was winded up around 1.30. Around the same time, The faculty from college
has arrived at Purna and I have went and picked up him, to the facility. We ha discussed about the
progress of internship, atmosphere and nature of works in PHC as well as he took a look at my progress
of Individual assignment. He has given me few suggestions and clarification as well. We have left
together for parbhani.

Today both the MOs were absent ,as one had went for Anganwadi supervision and the other MO was on
sick leave. As my internship partner is bed ridden, its quite impossible for me to interact with any other
staffs of the PHC. Most of them speak Marathi only, few of them speak hindi, which is quite mixed with
Marathi words I assume. As I am very weak in speaking hindi, and comprehending by hearing, the day
was mostly spent inactive. I observed the Junior Lab technician collecting blood samples, and few
prepared smears.

Today I travelled from purna along with a electrician, who has came with me to the PHC for repairing
the Operation theatre fumigation machine, which was not working. After the family planning program,
The OT was not fumigated. Once he repaired it, MO described me the procedure for fumigation of OT.
Its done with formaline liquid. The machine is just a storage tank with a fan on top of it. Once we fill the
cabin with the liquid and turn on it, the fan will exhaust the formaline liquid. The formaline will easily
turn in to vapur . It has an unpleasant smell as well eye irritating nature. One pregnant lady has reporte
with very low Hb . The case has been reporting for the second time in past few weeks. The patient is not
seeming to take proper nutrition or IFA tablet. The MO and ANM had to verbally force her to get
admitted in the facility. She has been given Iron sucrose as I/V and she was forced to stay there for 2
days. MO took a look around the post operative patients of Family planning program. He told that, the
loss of wage for the bystander is a critical issue for them, hence no patients prefer long hospital stays.
