Integrative Narrative of Theory of Treatment and Logic Model Assignment

Description
In this assignment, you are asked to develop a Logic Model and align it to your Theory of
Treatment (ToT) in a narrative summary for your POP intervention study.

Assignment Components:

1. Introduction
a. Clearly articulate your POP, salient needs assessment findings and your
proposed intervention. (2 paragraphs)

2. Logic Model: Specify the logic model for your POP intervention by clearly articulating
the inputs, activities, participants, outputs, and outcomes (short, medium, and long-
term).
a. Develop a figure/model of the logic model using one of the templates provided.
b. Discuss the logic model in narrative form with literature support (Length: 1 page)

3. Comparison of ToT and Logic Model: Compare your Theory of Treatment and Logic
Model for your dissertation intervention. How is your theory of treatment realized in your
logic model? To what extent are these two models aligned? Where are they not in
alignment? (Length: 1 page)

4. Logic Model Revision: Discuss how you need to revise your logic model in light of the
theory of treatment. If you believe that your logic model does not need revision, provide
support and justification for this position. (Length: 1 page maximum)

Your final product should be no longer than 3.5 pages of narrative and must include two figures
(Logic Model and Theory of Treatment). Follow the standard guidelines for APA formatting. The
figure, title page, abstract, and references do not count toward the expected page limit.

This assignment will be due at the end of session 3.
