MOTIVATION RELATIONSHIP WITH MEDICAL COMPLIANCE WITH

HYPERTENSION PATIENTS IN PONKESDES JADI SEMANDING SUB-


DISTRICT

Abstract

Hypertension is a disease that must always be controlled so that complications do not occur which can lead to
death. The purpose of this researchknows the relationship between motivation and treatment compliance inhypertensive
patients at Ponskesdes Jadi Semanding District, Tuban Regency

Research dCorrelational Analytical essays with a time approach case control. This sample was taken using
simple random sampling with a total of 24 respondents. data collection using questionnaires and secondary
data. Statistical test usingContingency Coefficient test.

Results obtained hypertension patients who adhere to treatment are found more in respondents who have weak
motivation, namely 100%. Statistical test results showwith a significance level of α = 0.05, the value of Phi = 0.001 is
obtained where 0.001 <0.05 means that there is Relationship Motivation of hypertensive patients with treatment
adherence to hypertension patients. It is recommended to educate officers that even though they do not feel dizzy, they
must be diligent in seeking treatment.

Introduction
Hypertension or high blood pressure is a problem that is often found in people in developed
and developing countries. WHO data shows about 1.13 billion people in the world have
hypertension. Therefore hypertension needs to be detected early with regular blood pressure checks
(Sidabutar, 2009). The lack of motivation of health workers will have an impact on hypertensive
patients to be able to cope with recurrence or regular treatment (Dinkes, 2018).
There are several factors that can affect the compliance of hypertensive patients in medicine,
namely lack of education, accommodation, family support, interaction of health workers and
motivation (Yulike et al, 2017). One of the efforts that can be made to lower hypertensive morbidity
is to treat or take antihypertensive drugs, but still few people with hypertension are treated regularly
(Dinkes, 2018).
Motivation for treatment for hypertension sufferers until now is considered a common
disease so many of the people feel themselves still healthy because they think that hypertension is a
disease that does not exist. So that efforts that can be made to improve treatment compliance in
hypertensive patients can be done with motivation from themselves, family support, mentality in
medicine, patient knowledge level and follow the activity of attention check in the local area
posbindu. Based on this, the researchers wanted to examine "motivational relationship with
hypertension patient care compliance in poskesdes so sub-district semanding tuban district in 2020"

Method
This research uses observational methods with correlational analytics design. with the time
approach case controlresearch respondents namely hypertension patients in ponkesdes So
Semanding Sub-district as many as 24 respondents determined using Simple Random
Sampling technique.. Independent variable Patient motivation dependent
variable is Treatment Compliance.. Retrieval of data with questionnaires and skunder
data. Statistical test using Contingency Coefficient test.
The test results showed a relationship between Motivation and Drug-Related Kepatuahan
i.e. with the level of meaning α = 0.05 obtained phi value = 0.001 where 0.001 < 0.05 then H1
is accepted meaning there is a Motivational Relationship of Hypertension patients with Hypertension
Patient Treatment Compliance in Ponkesdes So Semanding District Tuban Regency, there is a
strong correlation evidenced by the value of c = 0.608.
 
Results
Characteristics of hypertensive patients include Age, Gender, Education and Occupation.

Based on table 5. 1 can be found that almost half of respondents aged 61-70 years old number 11 (45.8%)
%) a small percentage aged 51-60 years old number 5 (20.8%).

Based on table 5. 2 it can be known that the majority of Female respondents are 14 (58.3%) and almost half
are male 10 (41.7%).

Based on table 5. 3 it is known that the majority of primary school respondents' education is 17 (70.8%) and
a small fraction of high school educated number 3 (12.5%).

Based on table 5. 4 it is known that most respondents work as farmers as many as 14 (58.3%) and a small


part works as a bricklayer as much as 2 (8.3%).

Based on table 5. 5 shows that half of respondents in Ponkesdes Jadi Semanding Sub-District have strong
motivations namely 12 (50%).

Based on table 5. 6 shows that the majority of respondents in Ponkesdes Jadi Sub-District Semanding Patuh
in Medicine are 14 (58.3%).

Based on the table of 5.7 of the above results, respondents who adhered to the treatment were almost entirely
obtained in respondents who had strong motivations of 91.7%, compared to respondents who had weak
motivation of 0%, while non-compliant respondents were found more in respondents who had weak
motivations of 100%, compared to respondents who had strong motivation of 8.3%.
 

Discussion
Patient motivation in Ponkesdes Jadi Semanding District Tuban Regency year 2020.
Half of hypertension patients in Ponkesdes Jadi Semanding Sub-District have Strong Motivation
which is 50% and a small part has moderate and low motivation which is 25.0%. According to standford,
there are three important points in the sense of motivation, namely the relationship between need, drive and
purpose. The need arises because of something felt by a person both physiological and

psychological. Encouragement is a direction to meet that need, while the goal is the end of a cycle of
motivation (Suarli & Bahtiar, 2013).
Hypertensive patients whose motivations are weak mostly confess because every dizziness they feel
has healed from hypertension then do not continue treatment. Hypertension is difficult to detect by a person
because hypertension has no specific signs/ symptoms. I don't want This is where the role of family is very
important. It is necessary to emphasize to family members to continuously monitor the development of
relatives suffering from hypertension related to symptoms that begin to subside and the condition of their
blood pressure. And for patients themselves should be aware of the importance of posthypertensive health
recovery. Because there are indeed no specific signs of symptoms related to hypertension. As well as being
very necessary for patients to maintain a regular diet and exercise.
Hypertensive patients who have weak motivation mostly claim the length of treatment
makes them unsure if they are able to treat for up to 6months. To overcome this can be done by providing
support to patients, and support can be done by health workers as well as from the patient's family, this is so
that the patient is passionate in carrying out his treatment and can increase the motivation in him to achieve
recovery.
Hypertensive patients who get more motivation in the male gender. While according to the age of
strong motivation there is at the age of 40-50 years. According to (Lestari, 2015) that physical factors are
factors related to a person's environmental condition and condition, including: physical condition of the
environment, state or health condition, age and so on. So it can be drawn conclusions from data related to
existing theories, that insight into awareness of the importance of health needs to be improved. Due to the
increasing age of a person, awareness of the importance of health tends to decrease.

Compliance of Medicine in Ponkesdes Jadi Semanding District tuban regency year 2020.
The majority of hypertension patients in Ponkesdes Jadi Sub-District Semanding Patuh in Medicine
are 58.3% and almost half are non-compliant by 41.7%. Of the 41.7% (10 people) who disobeyed treatment,
they quibbled if they did not feel dizziness did not continue treatment. treatment compliance is the behavior
to adhere to the advice or procedures of the doctor about the use of the drug, which was preceded by a
consultation process between the patient and the doctor or nurse as a healthcare provider (pratita, 2012).
Based on data from ponkesdes there are 10 hypertension patients who are non-compliant in
medicine, among them not continuing blood pressure control in the 4th month. Because of the ten
hypertension patients feel no dizziness and other signs of hypertension. Compliance is a behavior that is not
easy to carry out, because to achieve recovery from a disease requires compliance or regularity of medicine
for each patient. Patients considered compliant in medicine are those who complete the treatment process
regularly and completely uninterrupted for a minimum of 6 months up to 9 months (Ministry of Health,
2000).
Hypertensive patients who are compliant in medicine, who have a junior high education level of 75%
while hypertension patients who are not compliant in medicine have an elementary education level of
41.2%. According to Notoatmodjo (2005) factors that affect compliance are knowledge, motivation,
affordability of health care, support of health workers and family support. Most patients with non-compliant
hypertension have a low education namely elementary school, it can be known that knowledge is influential
in conducting treatment consistently compared to those who have a level of education on it, so it can be
known that education is very influential on drug compliance.
 

Motivational Relationship with Hypertensive Patient Treatment Compliance in Ponkesdes Jadi


Semanding District tuban regency year 2020.
Patients with dutiful hypertension were more likely to be found in patients with strong motivation,
compared to 91.7%, compared to patients who had weak motivation of 0%, while non-compliant patients
were more likely to be found in patients with weak motivations of 100%, compared to patients who had a

strong motivation of 8.3%. Meanwhile, according to Niven (2002) that factors, which affect compliance, are
the attitude or motivation of patients wanting to recover, confidence, family support, social support and
support from health workers.
The analysis in this study used the Contigence Coefficient test using SPSS Software For
Windows with a level of meaning α = 0.05 obtained a value of Phi = 0.001 where 0.001 < 0.05 then
H1 is accepted meaning there is a Motivational Relationship of Hypertension patients with
Hypertension Patient Treatment Compliance in Ponkesdes So Semanding District Tuban Regency,
there is a strong correlation evidenced by the value of c = 0.608.
From the results of the above study it is known that the motivation of hypertension patients in
Ponkesdes So almost entirely have a strong motivation with obedient treatment compliance because they
have hope to recover from hypertension is very high so that they do not feel pain again. On the other hand,
rosponden, which has a weak motivation category with non-compliant adherence, tends to have very
minimal knowledge that they consider the disease to be commonplace. And there is one hypertensive patient
who has a strong motivation but does not comply in treating the patient saying that it is too busy with his job
as self-employed so do not pay attention to when the control schedule is treating. Some choose not to control
their blood pressure due to lack of knowledge, busyness, and fear of the disease. It is recommended that
families play an active role in encouraging families suffering from hypertension to comply with treatment.
In the future, health workers should optimize again to patients with moderate and weak compliance,
both in terms of knowledge and in terms of motivation so that the treatment can run properly until the time of
treatment that has been determined and hope all hypertensive patients can comply in medicine and have a
strong motivation.
 
Conclusion
From the results of this study it can be concluded that:
1. Half of hypertension patients in Ponkesdes So Semanding District tuban regency year 2020 has strong
motivation.
2. Most hypertension patients in Ponkesdes So Semanding District Tuban Regency year 2020 are compliant
with medicine.
3. There is a significant relationship between the patient's motivation and the treatment of hypertension
patients in Ponkesdes So Semanding District tuban regency year 2020.
 
Advice
The advice given is as follows:
1. For The Next Researcher
It is hoped that researchers will be able to further develop this study using other variables with treatment
compliance in hypertensive patients.
 
2. For Institutions
The results of this study are usually used as curriculum development and improving the quality of their
students related to hypertension.
3. For Nursing Profession
The results of this study are expected to contribute thought in enriching and expanding about motivation
with the adherence of hypertensive patients.
4. For Respondents
It is expected that respondents can add insight into hypertension disease so that it becomes an influence
both for respondents and the local community.
