Basic Observations in Sick Children

The intent of this concise list is to aware parents of the most commonly occurring changes in
childhood disease.

These changes form the basis of diagnosing (i.e. naming the disease), and possible action (i.e.
treatment).
Any consideration of action should be based on these changes.

If unsure about the nature of the disease, or the proper cause of action,
or treatment, a health professional should be consulted, who will greatly
appreciate all information gathered by you considering the points below.

Timeline
When did this start? Was there a trigger? What was the succession of events? What is the
current situation? Did other children have similar complaints recently? Is there a known
epidemic?

Temperature
Normal body temperature: mornings: 36.6 Celsius, evening up to 37.5 Celsius
Increased body temperature: 37.50 -38.00 Celsius
Fever: 38.00 Celsius upwards to 41.5 Celsius.

Children: Temperature is taken at the bum. (-not the ear, as the internal ear is very sensitive)

Pulse, Heart, Circulation

Normal Rates:
0 years 140 / minute
2 years: 120/ minute
4 years: 100/ minute
10 years: 90/ minute
14 years: 85/ minute
Adults: 60 – 80/ minute
Seniors: 80 – 85/ minute

With every degree of body temperature increase, the pulse increases at least 10 strokes per minute.
Is the heartbeat: steady, intermittent, hard, soft, strong, weak, visible on chest
Is there coldness of single parts, blueness, paleness, redness?
Head
pains, fullness? Outer head: sweat?

Eyes
look your child in the eyes; is there normal reaction to light? (lens gets smaller when light shines into
it) are the eyes blood shot? Are the eyes glassy looking? Is there discoloration of the white? Is here
a discharge from the eye? Is the eye itchy? Dry? Burning?

Face
change in colour? swelling? Are there spots appearing? Touch the skin on your child’s
forehead. Is it cold? Clammy? sweaty? Hot burning? Dry?

Ear, nose
Pain, discharges, skin coloration changes? Swellings?

Mouth and Throat

Smell from mouth? Teeth? Gums? Dribbling from the mouth, dryness? Funny taste? Loss of
taste?
Tongue: coated? Swollen? Speech?
Tonsils: enlarged,? Red? White spots?

Chest, Breathing
Cough? Bringing up things? Tightness of chest? Painful breathing? Breath rate/ minute? Type
of breeding: deep / superficial / laboured, slow/ fast – smell of breath?

Digestion/ evacuations
Hunger? Appetite? Cravings - aversions? Thirst? Nausea? Vomiting? Cramping? Diarrhoea,
constipation? Urination frequency? urine smelly, dark?

Mood, mind
has the mood changed from normality? In which way?

Sleep
changes? Problems during sleep?

Energy
full of energy / listless?

General
Is there pain, swelling, irritation, inflammation, festering, bleeding, twitching, cramping,
redness, paleness, lack of muscle power, stiffness, uncontrolled movement, skin itchy, over
reaction to light, smells sounds, Reaction to cold / heat/ food / water? Is there drowsiness,
shivering, startling, vertigo, pains of all sorts in all different areas? Are there times when
things are better / worse? Are there circumstances which improve/ worsen the problem?
