Sub: Request letter for Name Correction

Dear Sir,

Recently I got my father's death certificate from the Government Hospital . My father's name
on the certificate is misspelled. My father's correct and complete name is ​(UTHAYAM)​ but it
is written on the certificate with wrong spellings as ​(UDAYAKUMAR)​. I request you to please
issue a new birth certificate with correct spellings (Name with correct spelling) as soon as
possible. I need to submit my Father Death certificate with the admission form next Day
