AFFIDAVIT

NO OBJECTION CERTIFICATE WITH CONSENT

I Shri, Jaymin Ajay Mistry son of Late Shri Ajay Dhirajlal Mistry,
aged 21 years about, Occupation Student, residing at, F-13, Shri Krupa
Apartment Karjat Road Near Kali mata mandir, Katrap, Badlapur east, Dist.-
Thane , State of Maharashtra state on solemn affirmation as under:
1. That Late Shri Ajay Mistry is my Father, his date of death was on
09/05/2021, his place of death was at Thane Civil Hospital, he also
possessed Aadhar and its number is 4677 5837 8124
2. We certify that, Late Shri Ajay Dhirajlal Mistry was a bonified member of
our society. On that account he is entitled to hold and occupy F-13, Shri
Krupa Apartment Karjat Road Near Kali mata mandir, Katrap, Badlapur
east, Dist.- Thane and 5 shares which are issued in her name bearing Share
Certificate No 13 ,Shares No 61 to 65 ( both inclusive ). Our Co-operative
society is registered under the name and style of The Shri Gajanan Co-
operative Housing Society Limited ( Shri Krupa Apartment ) registered
under the Maharashtra Co-operative Housing Socities Act 1960, bearing
registration no /TNA/ ULR/HSG/(TC)/8430/96-97/year 1996.
3. Late Shri Ajay Dhirajlal Mistry expired on 09/05/2021 at Thane leaving
behind him/her all the following legal heirs:
1. Smt Purnima Ajay Mistry (Wife) age 47 years about
2. Shri Jaymin Ajay Mistry (Son) age 21 years about
4. That the persons mentioned hereinabove are the only heirs of the deceased.
They are entitled to inherit singly/jointly the aforesaid shares held by the
deceased.
5. I do not desire to make any claim of title of the said shares held by the
deceased.
6. I hereby agree to renounce all my rights existing as well as they may accrue
to me in future in respect of the aforesaid shares.
7. I declare that I have no objection whatsoever in transmitting the said shares
transfer, assign , alienate said shares with rights over the said flat to consent
in writing of the flat in the name of my mother Smt Purnima Ajay Mistry.
8. That my father possesed Property Card no 5000782 old property tax no
283/13 transfer in his name and after the death of my father I would like to
give my whole consent to transfer the property in the name of my mother i.e.
mother Smt Purnima Ajay Mistry.
9. That we have not created any charge / mortgage over the said flat below the
said building and there is no encumbrance on the said flat.
10.That this affidavit is sworn into obtain Transfer of Property tax from the
competent authority i.e. Kulgaon badlapur Muncipal Corporation, Kulgaon ,
Badlapur East. 421503
I/we hereby state that whatever is stated herein above are true to the best of my/our
knowledge and I also acknowledge that I am aware that if I have stated any false or
wrong information which would be an offence and is also punishable under the
Indian Penal Code section 199 , 200 and 193.
