NAME AFFIDAVIT
I, Pramila Thapa, daughter of late Jeewan Thapa, aged about 53 years, by faith Hindu, resident of Sombaria
Bazaar, under Soreng District Sikkim , P.O & P.S- Sombaria, West Sikkim, do hereby solemnly state and
declare on oath as follows:-

1. That in the Mutation Certificate of my father that his name has been recorded as Jiwan Thapa, bearing memo
no. 6518/SDO/SRG, dated 10.02.1994, which has been issued by Office of the Sub-Divisional Officer Soreng,
West Sikkim.
2. That in the School Transfer certificate of my father that his name has been recorded as Jiwan Thapa
bearing serial no. 91492, which has been issued by Head of the Institution, headmaster, Government
Senior Secondary School Sombaria, on dated 21/07/1995.

3. That in the Death Certificate of my father that his name has been recorded as Jeewan Kr. Thapa,
which has been issued by Sunrise Nursing Home Pvt. Ltd. who expired on 20/02/2010 at 3.56 PM,
certificate Issued on 20/02/2010.

4. That in my Age certificate that my name has been recorded as Pramila Thapa and father’s name has been
recorded as Jeewan Thapa, bearing memo no. 358/I/BAC/DARAMDIN, which has been issued by Office of
Block Administrative Centre Daramdin, West Sikkim, Government of Sikkim dated 18.01.2022
5. That in my School Transfer certificate my name has been recorded as Pramila Thapa and father’s name
has been recorded as Jiwan Thapa, bearing serial no. 35450, which has been issued by Head of the
Institution, Government Senior Secondary School Sombaria on dated 26.11.2015.

6. That in my Aadhaar Card that my name has been recorded as Pramila Thapa my father’s name has been
recorded as Jeewan Thapa, bearing Aadhaar Card No. 8561 3989 9985, which has been issued by Unique
Identification Authority of India.

7. That in my Voter Identity Card that my name has been recorded as Pramila Thapa and my father’s name has
been recorded as Jeewan Thapa, bearing identity Card No.GCJ0089987, which has been issued by Election
Commission of India.
8. That in my unmarried Certificate that my name has been recorded as Pramila Thapa and my father’s name has
been recorded as Jiwan Thapa, bearing memo no. UMC-SK/2023/01097/B-DAR, which has been issued by
Block Administrative Centre Daramdin, Soreng, Government of Sikkim dated 22/06/2023.

9. That this affidavit is sworn to declare on Oath that the person by names Jiwan Thapa, Jeewan Kr. Thapa, and
Jeewan Thapa is also same and one identical person i.e. my father.

10. That this affidavit is made bonafide.
