Memo: Bed rail entrapment

Due to a recent incident involving a patient’s limb becoming trapped in a bed rail, staff are
requested to read the following from the Division of Quality Assurance (DQA).
Entrapment is when a patient becomes caught between a bed rail and mattress or within
the rail, and also includes any attempt to climb over the bed rail. Such instances can lead to
asphyxiation, strangulation, chest compression, fractures and serious circulatory
impairment, head injuries and subsequent death. The DQA encourages a review of practices
for initiating the use of bed rails and suggests that each patient be assessed individually. Bed
rails may serve to both restrain and to aid independence in mobility and repositioning.
Those at greatest risk of harm are the cognitively impaired, those of low body weight, or
those who have uncontrolled, poorly coordinated or restless movements.
