Slang and abbreviation use

To all staff: Please take time to read the following. For any questions or clarification, please
see your team leader.

It has come to our attention from several emergency rooms that many emergency medical
services (EMS) narratives have taken a decidedly creative direction lately. Effective
immediately, all members are to refrain from using colloquial terms and non-standard
abbreviations to describe patients and their conditions. Use of such language can cause
confusion and may mislead attending physicians, thereby increasing the likelihood of error.
This places patients, colleagues and the hospital at an unnecessary and unacceptable level
of risk. From this point forward, please use correct and accepted terminology for all care,
including patient admission, discharge or transfer.
