AFFIDAVIT OF UNDERTAKING

We, ___________________ and _________________, of legal ages, married, Filipino

citizens, and residents of _______________________________after having sworn to an oath in
accordance with law do hereby depose and state that:

1. We are the parents of Elah Sarelle L. Perez who is a delegate for chess secondary girls
division of Region IV-A in the forthcoming Palarong Pambansa to be held in Vigan City on
April 16-20, 2018;

2. Our child will not be joining the delegates on their scheduled trip to the aforesaid
venue on April 11, 2018;

3. She will also be living outside the quarters/camp assigned for the delegates of Region
IV-A;

4. We undertake to take full responsibilities over the person of our child during the
course of the aforesaid Palarong Pambansa;

5. We also understand and undertake to present the child during her scheduled games
at the assigned venue; and,

6. We are executing this affidavit to attest to the truth of the foregoing statements and
for all legal intents and purposes that it may serve.
