To Whom It May Concern:

This is to certify that Elvira N. Magbanua legal age, female, Filipino Citizen,
Married and resident of Barangay Inamotan, Manaoag Pangasinan come upon
completion of health protocols.
Per LGU’s COVID-19 health security guidelines she will undergo upon her return
to the municipality the required 14 days quarantine at home or in LGU facility upon the
advice of the municipality health officer.
Issued this 29th day of September 2020 as a requirement in the issuance of
medical certificate and LGU’s certificate of acceptance.
