Title: Develop an information sheet
Performance Objective:
Given one Learning outcome you choose/assigned to you from your session
plan, you should be able to develop an information sheet for all the contents of the
learning outcome following standard format and guidelines.
Equipment: PC with internet connection, CBC, TR, Session Plan
Steps/Procedure:
1. Search for the information sheet format in the PTS folio template.
2. Based on your session plan choose one Learning outcome.
3. Develop Information sheet of all the knowledge contents of your chosen
Learning Outcome.
4. Follow the standard format in the PTS folio template.
5. Be sure to develop contents towards the attainment of the main skill to be
mastered.
6. Prepare a self-check with answer key
7. Edit footer
8. Evaluate your work using the performance criteria checklist.
9. Present your output to your facilitator through google drive shared to your
facilitator’s email (tmlachica@tesda.gov.ph)

1. Information sheet is free from violation of copyright law .
2. Document contains information essential to the attainment of
the learning outcomes.
3. The title of the sheet gives some idea of the coverage of the
sheet.
4. Approach in terms of content and presentation is appropriate
to the interest and reading level of the trainee.
5. The layout text and drawings are attractive in appearance
and legible.
6. Uncommon terms are marked for further defining.
7. Acknowledgments are made per copied part of the sheet
8. Reading level matches that of your trainees
9. Paragraphs are short.
10. Sentences are concise.
11. Illustrations are located to the side or just below the
sentences referring to the illustrations.
12. Information sheet is not too short but not too long.
13. Prescribed format is followed with the following parts:
-Smart learning objectives
-Introduction
-Body / Text
-Self checks
-Answer key
14. Self-checks assess the knowledge content in the information
sheet.
15. There is a model answer or answer key.
16. Edited footer.
