School, Araling Panlipunan Department since May 15, 2014 up to the present holding a Teacher
II position. During this period, I can confidently say that I have conscientiously and diligently
performed the duties and responsibilities assigned to me.
This is to express my intentions to apply for the position of Senior High School Teacher
under the Humanities and Social Science Strand at any station available in the Division of
Manila.
Attached herewith is my Personal Data Sheet detailing my personal circumstances for
your evaluation. Other personal documents such as transcript of records and certificate of
training are available upon request.
Work wise, at present I am rendering six hour – workload daily and classroom adviser for
the past five years. I am currently enrolled for my thesis writing subject and waiting for my
schedule to defend my papers.
With nature of my work, I am willing to perform my duty beyond my working hours if
needed or requested.
