JUDICIAL AFFIDIVIT OF IMELDA P. ANTAYIN

This Judicial Affidavit of Imelda P. Antayin, is executed to serve as her direct testimony
in the instant case.

This Judicial Affidavit is being offered to prove:

A) All the allegations in the Petition including all annexes appended thereto and

which were already marked as exhibits during the Pre-Trial of this case;

B) All other related matters, facts and circumstances relevant and material to this
case.

This Judicial Affidavit was taken at the office of Atty. ________________ at Unit 1152,
VG Tower, Corona Village, Antipolo City.

Questions were propounded by Atty. ________________ and these questions are

numbered consecutively and each question is followed by the answer of the witness.

1. Do you swear to tell the truth and nothing but the truth?

I do.

2. Are you aware that you may face criminal liability for false testimony or perjury if
you will not tell the truth?

I am.
 3. Please state your name, age address and occupation?

I am Imelda P. Antayin, 43 years old, married, and residing at 505 West Tower
Condominium, Makati City.

4. Do you know the Respondent in this case, Anthony P. Antayin?

Yes, he is my husband.

5. Where were you on January 2, 2020 at about 10 o’clock in the evening?

I was at home together with my husband and the children

6. Do you own a white fortuner SUV bearing the plater number of VBE 1234?

Yes.

7. Was this fortuner car in your house garage at that evening?

No, it was borrowed by Anthony’s cousin.

8. What happened next?

My husband went to the garage to check if his cousin went home already since
their agreement was to return the car that evening

9. What happened next, if any?

We went to bed and was not able to wait for his cousin to return on that night

10. What happened next, if any?

The car was returned on the evening of January 3, 2020 and left without any
notice from us.

11. Do you want to add anything?
