Fact-Finding Instrument (Research Instrument)

An adapted Survey Questionnaire on Blended Learning and School Operations (SQBLSO) During the COVID - 19 Pandemics
was used as instruments in this study.
Survey Questionnaire on Blended Learning and School Operations (SQBLSO)
The survey questionnaire is the instrument planned to resolve the level on blended learning and the implementation of it during
COVID – 19. There are thirty reliable statements related to the learning convenience and flexibility on how effective blended learning
and school operations during this time of pandemic. Each level of effectiveness has scores which the teachers/students are responsible
to respond by putting a check under the appropriate column of the options ranging from Very Effective (5), Effective (4), Moderately
Effective (3), Slightly Effective (2) and Not effective (1).
The total score obtained in each statement was converted into equivalent code, qualitative description and quantitative
statistical results confirmed the level on blended learning and school operations during COVID-19 pandemic.
Data Gathering Procedures/Methods
In this research, data gathering procedure is succeeded by six stages: development of action plan, identifying the sample,
development of teacher-made questionnaire, seeking permission to conduct survey, performing the survey, and collection of the
accomplished questionnaire.
Stage 1: Development of action plan
Action plan in this research is integrated of aimed goals or campaigns to be done in a week. Aimed goals were instructed
during session to orient the other researchers concerning the plan.
Stage 2: Identifying the sample statement
Sample statements were chosen based on the learning convenience and flexibility which the teacher – researcher conducted.
Stage 3: Development of teacher-made questionnaires
Teacher-made questionnaires were established based on the pre-determined blended learning and school operations during
COVID – 19 pandemics of the students and teachers. The survey questionnaire has thirty (30) statements that corresponds to a specific
learning convenience and flexibility on blended learning and school operations, such as blended learning brings convenience at the
pupil’s home, gives safety to pupils to learn at their home, enables the pupil to access the materials from anywhere at anytime,
provides more time to learners to study the material/s, can increase retention rate and etc.
Stage 4: Seeking permission to conduct survey
As part of the methodology, the researcher customarily writes a letter of request to plan a survey in the participating public
school, Kapitan Eddie T. Reyes integrated School (KERIS) to the Office of the Schools Division Superintendent of Taguig and
Pateros on November 20, 2020. Permission letter will be released on November 20, 2020.
Stage 5: Surveillance (Administration) of the survey
Surveillance of the survey will be done on November 3 – 9, 2020 anywhere and anytime using online google forms via internet
with the instruction of the assigned researcher. The survey took almost a minute per person.
Stage 6: Compilation (Collection) of the accomplished questionnaire
Compilation on the accomplished questionnaires was done between the given date as the survey was administered.

Ethical Considerations (Honest application or examination)


Collaboration in this study is independent or voluntary with explicit permission from the respondents and at the same time,
with the return of the permission survey form from them. The privacy of respondents’ information is anonymously unknown. There
are no dangers associated for being a part of this study.

Statistical Treatment of Data


In order to know the statistical treatment of data on blended learning and school operations during COVID-19 pandemic just as
measured by the (SQBLSO), the respondent’s feedbacks are individually added and totaled. The total score in each statement of the
SQBLSO were converted to its equivalent code and qualitative description. The criteria for classifying blended learning and school
operations during COVID-19 pandemic are shown on Table 1.
