    We are the Grade 12 students of the ABM strand enrolled in Inquiries, Investigations, and
    Immersion. We are required to conduct a study as part of our requirement.

    Our study is on “Awareness and Perception of Notre Dame University Senior High School
    Faculty and Staff on Income Tax.”

    In line with this, we would like to ask permission from your office to allow us to conduct our
    study by giving the survey questionnaire to the SHS Faculty. All information provided will be
    kept in utmost confidentiality and would be used only for the study.
