    To apply for financial assistance, students must submit the following documents: 1) Form 138 for freshmen or transcripts for last 2 semesters for current students. 2) A certificate of indigency from their local barangay. 3) Their parents' certificate of tax exemption from the Bureau of Internal Revenue. 4) A good moral certificate from their last school attended. 5) A completed STUFAPS application form which can be downloaded from the CHED website or obtained from the school office. 6) One 2x2 ID photo.
