Special Education Forms - YCDSB - Reference

SE1 White OSR History Used to create a summary of a student’s educational history including access to programs and services

SE2 Yellow Meeting Record Documents purpose and outcomes of a case conference or parent meeting

Every person listed on the SE2 must receive a copy of the meeting record

SE3 Green Consent for Consultation with Board Staff Updated annually or each time a new board support personnel is going to be involved in supporting the
child (i.e., consultation)

SE4a Green Consent for Initial Academic Assessment This must be signed before completing an initial academic assessment; does not need annual updating,
however courtesy call to parents before assessment is suggested

SE4 Goldenrod Academic Assessment Every student receiving Core Res. support must have one or the other updated annually (typically
around IPRC review/SE5 anniversary)

SE4R Goldenrod Academic Assessment Review

SE5 Green Consent for Special Education Support Services / Must be updated annually if a student has not been identified as exceptional by IPRC and continues to
Consent for Core Resource require special education services

Invitation White IPRC Invitation Sent to parents with details of IPRC meeting, must provide a minimum of 10 working days’ notice. Initial
IPRC, also include a copy of the Parent’s Guide to Special Education

SE6 White IEP Individual Education Plan

SE7 Blue IPRC initial This is the initial IPRC for a student; happens at the board level and is generated by the Program
Consultant

SE8 Blue IPRC review Annual IPRC review happens at the school level and is generated by the Core Res. Teacher; signed by
principal and two other committee members

SE10 Green Parent Consent to Waive IPRC Review Parents/guardians use this form to waive the annual IPRC Review when the student’s Identification and
Placement will remain the same for an additional year.

S7 Green Consent for the Exchange of Information Anytime outside information is shared with the school or the school is being asked to share with an
outside party, parents are to sign an S7 giving consent for confidential information to be shared

Decision to Decline Green Parent Decision to Decline Special Education Program Completed when parents decline Core Support, support of the area team.
and/or Placement and/or Service
Please note when a student is being demitted from Special Education, 2 forms must be generated :
1) Decision to Decline form
2) SE5 form with Decision to “Decline Special Education checked off.”
