MOVE OUT AGREEMENT

This AGREEMENT is made and executed this _____day of________________at Brgy. Don Bosco,
Parañaque, by and between:

ALBERTO BAGALAY, hereinafter referred to as “LANDLORD” and JILLA CASUMPANG-ELIAS,

hereinafter referred to as "TENANT" concerning the lease property located at 221A Hongkong
St. Better Living Subdivision, Brgy. Don Bosco, Parañaque City.

NOW, THEREFORE, IT IS HEREBY AGREED BY AND BETWEEN THE PARTIES HERETO AS FOLLOWS:

1. Tenant has agreed to vacate the premises and is signing this agreement to that end.

2. Tenant agrees to vacate the premises on or before __________________hereinafter

referred to as the “Move Out Date” and leave the premises in a clean condition with
no damage to the premises, reasonable wear and tear excepted, with no one else left
at the premises.

3. Tenant shall deliver possession of the premises to Housing Provider by giving the keys
to the premises to Landlord.

4. Landlord has decided to forego payments for the month of March and April in the
beginning of the lockdown during this ongoing pandemic.

5. This Agreement is a settlement of both parties agreeing that the two-month security
deposit amounting to Twenty One Thousand Pesos (21,000.00) will be used as rental
payment for the month of _______________and ______________ granted that there is
no damage to the premises and all utility bills are settled.

6. Each of the parties acknowledges the attached receipt of payments and has arranged
that the unsettled months that will not be covered by the security deposit, amounting
to __________________ will be paid in staggered to ____________________________
every ______of the month until it has been fully paid.

7. All parties below signed hereby affirm receiving a copy of this agreement.
