AFFIDAVIT OF LOSS

I, Jocelyn B. Alilao, of legal age, married, Filipino, and a resident of PA

388, Upper Wangal, Wangal, La Trinidad, Benguet Philippines, after having
been duly sworn to in accordance with law, do hereby depose and state THAT:

1. Sometime on 2004, I was issued a Business permit for my Eatery

business located at Wangal, La Trinidad Benguet, together with other
documents;

2. Since then, the said business permit kept inside the eatery/in my
place of business, however sometime on January 2004, I closed my
business eatery due to bankruptcy;

3. On July 2020, I applied for scholarship for my daughter at the

Department of Health (DoH) Baguio City, as one of the requirement of
the said application for scholarship I need to submit a Tax exemption
with the DoH. I then immediately searched for my papers and/or
business documents before. Purposely to secure a certification of no
business with the treasurer office of La Trinidad Benguet. To my
frustration, when I diligently searching and carefully look for it, I
notice that the said document particularly my business permit was
missing;

4. I therefore immediately tried to search the said business permit inside

our house and evenly asked my family members, but despite my
earnest efforts, I was unable to find it. In view of the foregoing, I
strongly believed that same has been inadvertently lost without any
deliberate intent on my part whatsoever. Therefore, I considered it lost
and beyond recovery;

5. Said business permit has neither been used for any illegal activities
nor confiscated by any government authorities but due to the afore
cited reason;

6. Finally, I am executing this affidavit to attest to the truthfulness of

the foregoing facts and for all legal intents and purposes it may serve
well.

IN WITNESS WHEREOF, I have hereunto set my hand this 13th day of
