1.0 PURPOSE

a) The purpose of these guidelines is to prevent accidents, injuries, and damage to equipment, loss of
process or environmental damage during operations.

2.0 SCOPE

a) These guidelines define the minimum safe operating requirements and describe the following for
Progenitive Services Limited: – Rig-Up & Installation of Coiled Tubing Lift-Frame & Injector Head on Semi-
Submersible Rigs

b) These guidelines do not define specific equipment specifications nor is it to be considered an operators
manual.

c) If Client or Regulatory agencies require additional controls, these procedures must also be followed in
addition to these guidelines.

d) In case of contradictions or conflict between these guidelines and other client or regulatory documents,
the applicable standard should be agreed by all parties prior to the start of operations. This guideline
should be followed if there is any doubt which procedures should be followed.

3.0 RESPONSIBILITIES

3.1 The Well Operations Supervisor:

Responsible to comply with this standard and provide the following items

 Correctly equipped and maintained equipment


 The competent personnel for safe operations
 The appropriate training according to this standard so that employees understand their
responsibilities.

3.2 The Well Operations Engineer:

Responsible for collecting all necessary information to plan and perform any coiled tubing job involving
Rig-Up & Installation of Coiled Tubing Lift-Frame & Injector Head on Semi-Submersible Rigs. These
persons must communicate all information to the Job Supervisor and ensure the following tasks:
 Know and understand the requirements of these guidelines and any other PSL standards.
 Co-operate with the client to ensure all jobs comply with PSL procedures and any client or regulatory
requirements.

3.3 The Job Supervisor:

Must comply with all procedures and must not allow PSL employees to be exposed to risk levels that are
assessed as unacceptable.

 Know and understand the requirements of these guidelines and any other PSL standards.
 Co-operate with the client to ensure all jobs comply with PSL procedures and any client or regulatory
requirements.

3.4 The CT Crew:

Must comply with all procedures. The crew will undertake any task that the Job Supervisor delegates to
proceed with the operation. The crew will ensure that they are fully aware and conversant with any task
they have been asked to perform. At no point must the crew members expose themselves to any
unacceptable levels of risk. If any changes occur during an operation or task being performed, the crew
members must stop immediately, identify any additional hazards and ensure any resultant risks are
controlled within acceptable limits before proceeding. If further clarity is required, authority to proceed
must be approved by the relevant supervisor.

 Know and understand the requirements of these guidelines and any other PSL standards.
 Co-operate with the client to ensure all jobs comply with PSL procedures and any client or regulatory
requirements.

3.5 General QHSE Responsibilities

a) It is the responsibility of the Groups project / job - Engineer, Supervisors and/or Operators to ensure that
before the implementation of job specific procedures, that they comply with the Groups policy to work in a
safe and effective manner under the H & S at work Act 1974 and it's Duty of Care or relevant governing
policy of the work place.

b) Therefore, before and during the implementation stage of operational activities, it is required that all
safety precautions provided are undertaken and enforced to safeguard the integrity of personnel,
equipment and property.(Reference is to be made to QHSE policies and procedures).

c) Mandatory considerations are to be addressed as applicable for the scope of work and site location. These
Mandatory considerations may include but are not limited to:

 Hazard Identification for the tasks to be undertaken.


 Risk Assessment for the entire process and the safe implementation of the control measures.
 PUWER assessment of operational equipment.
 LOLER assessments for operational requirements.
 COSHH Assessments for the safe identification and use of chemical substances.
 Manual handling assessments to prevent lifting injury.

d) Working at height must also be considered along with the possibility of Dropped Objects and/or working
immediately adjacent to or below open deck hatches i.e.

 Keep all unsecured tools and equipment a safe distance from open hatches.
 Hard barriers c/w “Kick Boards” are to be erected around open hatches and a suitable hatch cover is
to be installed where practicable and safety harness worn when required
 Work should not be carried out immediately above or below personnel
 For land operations, a cellar entry permit / gas test must be obtained prior to executing any relevant
operation. A rescue plan must be incorporated in any relevant risk assessments.

NB: Every individual must identify the hazards associated with their work and take time out for
safety. Where there is any doubt about their ability to carry out the job safely, they must seek
guidance from their supervisor.

e) Risk Assessment(s) are to be reviewed prior to and during the operation: Especially if any deviation from
any work programme is noted.

f) The above issues can be addressed and recorded in Risk Assessments and 'Tool-Box Talks’ prior to
appropriate stages in a process e.g. at shift change, when any new members join the crew and when
conditions change for any reason.

4.0 PROCEDURES

4.1 Policy, Personnel, Training and Qualifications

a) PSL Policy requires that this standard be followed for all Rig-Up & Installation of Coiled Tubing Lift-Frame
& Injector Head on Semi-Submersible Rigs. The Well Operations Supervisor and Coiled Tubing
Supervisor/Job Engineer, in conjunction with the Well Services QHSE Engineer, must prepare and approve
additional procedures to manage "risks" during Rig-Up & Installation of Coiled Tubing Lift-Frame &
Injector Head on Semi-Submersible Rigs.

b) The following guidelines describe the standard procedures to be followed for all Coiled Tubing Operations
that involve Rig-Up & Installation of Coiled Tubing Lift-Frame & Injector Head on Semi-Submersible Rigs.
The range of services covered cannot be exhaustive, and so job engineers and supervisors should ensure
that any operations out with this standard have been thoroughly planned and risk assessed and any
remedial steps and contingency plans have been considered for the operation in question.

c) Where these operational guidelines contradict or conflicts with a client operating standard, the situation
MUST be highlighted and agreement with the client reached, on which standard will apply, PRIOR to
starting operations. This process may require to be documented on a CVI (Confirmation of Verbal
Instruction - Ref. PRO/P/011) to ensure clarification has been agreed.

d) The Well Operations Supervisor and the Well Operations Engineer for PSL Well Services Business Unit are
responsible for ensuring that enough people are assigned to the job to safely perform all tasks. The
amount of equipment, how the equipment will be installed and the length of the job, must all be
considered in determining manning levels. Personnel must have the correct training and skills/level of
competency for the job assigned.

e) The requirements above are the minimum requirements for Well Services operations. The Well Operations
Supervisor must ensure that the person assigned as the Job Supervisor has the skills and experience for
the job.

4.2 Safety Meetings

a) Prior to commencement of Rig-Up & Installation of Coiled Tubing Lift-Frame & Injector Head on Semi-
Submersible Rig operations, a safety meeting (or Tool Box talk) is to be held with all relevant personnel.
Discussions will include, but are not limited to, individual roles and responsibilities for the operation, with
emphasis on safety, job objectives, co-ordination of activities and lines of communication.

NB: Due to the inherent risks associated with inclement weather, an up to date weather report
should be obtained and referred to prior to the commencement of this type of operation. (Ref.
Section 4.5)

b) A location assessment can be done to identify any hazards or risks and the actions to control these risks
must be discussed at this meeting. (Ref: RISK Assessment Procedure - SAF/P/2.03 or clients equivalent).
c) After the equipment is rigged up and tested, a final toolbox talk is to be held before beginning any work.

NB: The Client representative should be invited to attend these meetings.

4.3 Preparation & Installation of Lift Frame onto Rig Floor

1. Inspect all equipment on arrival at rig.

NB: Reference must be made to the CTLF Safety Check-list (7314-036 – see section 4.7) and Risk
Assessments (RA01-017, RA01-018 & RA01-019, RA02-040) prior to lifting the CTLF onto the
Rig Floor

2. Install Lifting Nubbin.

3. Fit the winch basket to the securing points at the top end of the lift frame, connect hydraulic hoses,
utilising hose guides on the lift frame, and function test winches.

4. Disconnect Hydraulic hoses from control panel and winch basket.

5. Attach fall arresters as required and pull out line to ensure accessibility once CTLF is raised

6. Ensure the lift frame is correctly slung to minimise handling problems at the V door.

7. Guide the lift frame into the V door.

NB1: The catwalk tugger should be used at this stage to act as a tie back and tag lines should be used to help
steady the lift frame. The expertise of the Drill Crew should be utilised throughout this operation.
NB2: Extreme care should be taken at this stage to prevent damage to the winch basket.

8. Ensure the CTLF is correctly attached/latched into the drilling block utilising a 5” drill pipe elevator.

9. Pick up on the drilling blocks while supporting the bottom end of the lift frame with the crane.
Continue picking up with the drilling blocks and supporting the lower end of the lift frame with the
crane until the lift frame is hanging in the vertical position in the derrick or until it is necessary to
release the crane. Ensure stability with tuggers and sheaves if the crane is released.

10. Remove lifting gear from side of the CTLF and rotate to the correct position allowing the rig up of
injector head.

NB: Great care to be taken when releasing and lowering lifting slings to avoid injury to personnel

11. Install bails and elevators onto bottom of lift frame.

12. Reconnect the winch basket Hydraulic hoses to the control panel and winch basket. Tape the
hydraulic hoses to the outside of the CTLF to prevent damage when picking up the Injector Head.

13. For Man riding Operations ensure the correct man-riding harness and lanyard is fitted to the man-
riding tugger.

NB1: MAN-RIDING OPERATIONS SHOULD ONLY BE UNDERTAKEN AS A LAST RESORT

NB2: A MAN RIDING CHECKLIST (Ref SAF/P/5.18-2) MUST BE COMPLETED PRIOR TO ANY MANRIDING
OPERATIONS

NB3: Connection from the CTLF to the riser/flowhead will depend on specific installation
procedures and if the completion is to be landed using the CTLF.

4.4 Rig-Up Guidelines

1. Lift the BOP onto the rig floor and pull out the hoses from the control cab. Ensure that enough length
of hose to reach the injector head working position in the stack up has been laid out inclusive of
additional length in case of emergency disconnect (Check Emergency Disconnect Height with Driller).

2. Connect the hydraulic hoses to the BOP and function test the same.

3. Using CTLF Auxiliary or Injector winch, lift BOP's up into CTLF and make up onto surface test tree
riser.

4. Pressure test BOP shear seal rams

5. Lift injector head onto rig floor and secure with tuggers or chains. Exercise caution whilst
manoeuvring injector head through V-door.

NB: Tag lines should be used to help steady the injector head.

6. Connect all hydraulic hoses to injector head. Function test the injector head. Ensure that enough
length of hose to reach the injector head working position in the stack up has been laid out inclusive
of additional length in case of emergency disconnect (Check Emergency Disconnect Height with
Driller).

7. Pull out coiled tubing and stab into injector head as per standard procedure.

8. Pick up the injector head enough to make up the tubing end connector / MHA using the CTLF Injector
winch. (Ref. Drawing PSL01210)

9. Make up the tubing end connector / MHA, pull and pressure test.

10. Deploy the BHA into the riser through the BOP’s, utilising the CTLF auxiliary winch, support with a "C"
plate and safety clamp.

11. Pick up the injector head into the CTLF with winch, continue to pick up until it is possible to make up
last BHA connection. Lower the injector so that the final connection can be made up.

12. Make up last connection and remove "C" plate and safety clamps.

13. Lower the injector head onto BOP’s and make up quick union or JHS.

14. Keep the injector head stable using the winch but do not take the weight of the injector head.

15. Secure the injector head into the CTLF. The injector head should be secured from the crash
protection frame mountings to the uprights of the CTLF.

4.5 Guidelines During Bad Weather Operations

a) These operating parameters are to be used as guidelines only. Bad weather conditions affect all mobile
drilling rigs differently.

b) The decision to suspend or carry on with the operation will be made onsite by the coiled tubing supervisor,
the Toolpusher and the operating company representative.

c) Rig heave, pitch and roll all have to be taken into consideration during coiled tubing operations conducted
from a semi submersible drilling unit.

d) In order to minimise crew exposure and reduce excessive coiled tubing fatigue, the PSL Group implements
the following parameters as guidelines on mobile drilling units:

 Maximum heave during rig up of coiled tubing unit, change out of bottom hole assemblies and rigging
down of coiled tubing unit – No greater than 5ft (1.5m)
 Maximum heave during coiled tubing operations, RIH, POOH or any other work in well – No greater
than 10ft (3m).
NB1: These limits are for guidance only. The clearance between coiled tubing equipment (e.g Gooseneck etc.)
and the rig derrick must be considered.

e) If the weather is predicted to imminently deteriorate out with the parameters in d) above, the following
steps should be implemented.

1. POOH

2. If when at surface the weather is calm:

i. Close in xmas tree valves.


ii. Bleed pressure from riser and coiled tubing.
iii. Check the BHA.
iv. Review weather forecast.
v. Continue with operation if within limits stated in d) above.
3. If when at surface the weather is inclement:

vi. Close in xmas tree valves.


vii. Bleed pressure from riser and coiled tubing.
viii. RIH/POOH slowly at a controlled speed until weather abates to an acceptable level and/or
preparation is made to carry out step iv) below.
NB1: Keep all non essential personnel clear of area between coiled tubing injector on rig-floor and front of
coiled tubing reel

NB2: If perforating guns are part of BHA every effort should me made to keep the BHA below sea level during
this stage.

NB3: If coiled tubing logging reel is being utilised, ensure all power is turned off.

ix. To prevent excessive cycling of the coiled tubing at the reel, a loop should be made in the coiled
tubing between the C/Tbg. Injector and C/Tbg. reel by clamping the coil at the level-wind head
on the reel and POOH 5 to 10ft (1.5 – 3m).
NB: Prior to carrying out step iv) all hazards must be identified and adequate control measures
implemented to control resultant risks.
