his official duties 1800H - 0600H of January 01, 2021
around 2200H 1500H-0600HH

GENTLEMEN:

This is to formally inform you that you have committed an offense of sleeping on post. That you are caught in
the act of sleeping in sitting position at your designated post located at Loading 2 area, while the undersigned
with Mr. Glen Rostata corporate security Ninja van Philippines where conducting inspection within the area at
about 2200H 01 December 2021.

that your violation are very detrimental to our part since we have committed to our clients to deliver quality
service.

In view of the foregoing, you are hereby directed to explain in writing within 48 hours upon receipt of this DR
as to why no disciplinary action shall be taken against you for committing the above-mentioned offense. Failure
to explain within the prescribed period would be deemed considered as admission of guilt and waiver of your
right to be heard and thus you will be subjected to appropriate disciplinary action.
