of PALECO)

March 10, 2021 I answered through transaction slip the routinary treatment once certain
(1) individual applied for electric connection to PALECO.

August 16, 2021 Notice of Invitation from Fact Finding Committee for interview.

Membership Issue of Engr. Gonzalo Ong.

August 27, 2021 I received the Formal Charge.

This is in matter of complaint/ investigation on irregularities, falsification and
violation of procedures and rules on membership application/PMES of certain
Engr. Gonzalo Ong.

Sept. 02, 2021 My answer to the Formal Charge EDDCOM Case No. 2021 -01

My answer was address to Employees Deportment and Disciplinary
Committee (EDDCOM).

My supporting evidence includes the following;

a. ) My own transcription from my record during the FFIC investigation;

b. ) Sworn statement of Engr. Rey S. Lagan, the Area Chief of PALECO

Brooke's Point denying all allegations against me, and supporting by facts
the Engr. Gonzalo Ong had an one-on-one PMES with him for how many
times.

This sworn statement has not been considered / read and understood by the
EDDCOM members.

c. ) Evidence that it was not me who holds the hard copy for PMES
Attendance and I have not directly falsified the sheets which I don't have.

My complaint was submitted to the office of Acting General Manager

Bienvenida Tongol.

Sept. 22, 2021 Notice of Hearing from EDDCOM.

EDDCOM Case No. 2021 - 01.In the matter of complaint/ investigation on
irregularities, falsification and violation of procedures and rules on
membership application/ PMES of certain Engr. Gonzalo Ong.

Sept. 22, 2021 I submitted as instructed by EDDCOM after the hearing the Annex -D - 1
(7) and addition statement in contrast to the evidence submitted by Engr. Ian
Cyverlek Badua.

My supporting evidence includes the following;

a.)Screenshot of PMES Attendance sheet sent by Engr. Ian Cyverlek Badua

attesting that Engr. Gonzalo Ong was listed.

This is a manifestation In my belief that Engr. Gonzalo Ong was really on the
listed in PMES Attendance sheet.
