REFERENCES:

That while our Operation Officer Fernado Lauron Jr. conducting surprise post inspection, you
been caught dozing off in your post at 12th floor phase JPMC operation floor entrance area. Based
on the violation committed, it is clear violation of Benman Company Code of Conduct –
Category II “Serious Offense, Paragraph No. 8 – Carelessness or negligence in the
performance of one’s duty.

1. In view of the forgoing reference, you are hereby directed to submit a written explanation
within Twenty Four (24) hours upon receipt of this notice on why no disciplinary action
taken against to you for refusing to accept the order.

2. Failure to submit a satisfactory explanation immediate or within Twenty Four (24) hours
upon receipt hereof shall constrain this office to take appropriate disciplinary action
against you and waived your right to be heard.

3. This memorandum shall form part of your 201 File.
