RE: NOTICE TO EXPLAIN

This letter serves as a formal request on your side to provide the Management your written Notice to
Explain (NTE) involving an incident that happened last October 22, 2023 at the Negros Oriental Area.
The incident or concern that you were involved are the following:

It has come to our attention that there been significant discrepancies in the handling of certain
transactions involving clients in the Negros Oriental area. We take such matters seriously, as they directly
impact the trust and integrity of our organization.

On October 13, 2023. The Accounting and Finance Department sent out statements of account (SOA) to
clients in the Negros Oriental area through a messenger. On October 22, 2023 we have received
correspondence from one of our valued clients, specifically BOTICA GENESIS, expressing a concern
related to a sales transaction. This issue pertains to a specific entry in their Statement of Account (SOA)
that was received on October 13, 2023. According to the client's report, the sales transaction bearing
reference no. 0000001010, and involving a total amount of Thirty-Five Thousand Seven Hundred Two
Pesos (Php35,702.00), has already been settled in full through a cash payment. This payment was made
directly to Mr. Roberto P. Celis, who serves as the dedicated salesman for the Negros Oriental area.

Upon a thorough review of the said sales transaction in our system, it was noted that no payment had been
submitted by the client at our office. We take this allegation seriously as they represent a clear violation
of our company’s values and ethical standard . Your actions not only reflect negatively on our company
but also breach the trust that our clients have placed in us. Such behavior undermines the integrity of our
company and cannot be tolerated.

Hence, the Management will follow the disciplinary measures stipulated in the Company’s Code of
Conduct guided by its Policies and Procedures. It is known that the Company values its Employees and
wanted to hear each side of the story. Thus, allowing you to explain the alleged committed violations,
your explanation should contain clarification of your behavior and factual details that should provide the
Management proper information and judgement to justify whether the Management should impose the
necessary sanction or not.

The Management is expecting to receive your Notice to Explain (NTE) in hand writing, signed by You
not later than twenty-four (24) hours upon receiving this letter. Failure to cooperate and submit your NTE
will forfeit your chance to explain and defend your side. This will force the Management to proceed with
imposing necessary actions including legal remedies.
