Re : Notice to Explain and Preventive Suspension

Management received information that on or about 11:28PM and APRIL 29, 2021 that you
committed the following acts:

This act if proven to be true constitutes a violation of the Company Rules and Regulations.

 Refusal to obbey official orders from the Management

 Tahasang hindi pagsunod sa kautusan mula sa kumpanya o nakatataas lalo na kung
ito ay para sa kapakanan at ikabubuti ng kumpanya

In view of the above, you are hereby required to explain in writing within five (3) days or
seventy two (72) hours from receipt of this Notice why you should not be subjected to any
disciplinary action by the Company.

Furthermore, you have been placed under CODE OF CONDUCT F2/DISMISSAL to

commence upon receipt of this letter.

Kindly acknowledge receipt of this Notice by affixing your signature on the space provided
below. Your refusal to receive said Notice will be construed as a lack of interest on your
part to be duly informed as well as defend yourself on the matter. Your failure to submit the
written explanation shall be interpreted as a waiver of your right to be heard. In which case,
Management will proceed and conduct an investigation on the matter based on the
documents submitted and evidence gathered, and may accordingly impose the appropriate
penalty/sanction in accordance with the Company Rules and Regulations.
