EMPLOYMENT AGREEMENT WITH DEFINITE PERIOD

Ultimate Care Megaventures Corporation has favorably considered your employment with the following terms and condition:

DEPARTMENT : LOGISTIC DELIVERY

DATE OF EFFECTIVITY : AUGUST 16, 2023

EXPIRY DATE : JANUARY 28, 2024

OTHER BENEFITS : a. 13TH month pay


b. Basic Pay Php. 429.00
c. Allowance Php. 100.00 (Mindoro) - Php. 150. 00 (Bicol, Quezon, Marinduque
Cavite and Batangas)
1. It is hereby agreed and understood that the employment is with a definite period but on limited basis under a NO
WORK, NOPAY SCHEME.
2. Your employment shall be evaluated on or before the expiry date of this employment agreement as necessary for the
approval of your regularization of employment.
3. You are also subject to SSS, PHILHEALTH and PAG-IBIG contribution every end of the month.
4. The company may discontinue your employment before the above stated expiry date if after our evaluation your
performance is below the minimum standards.
5. For just and valid causes, or whenever no work is available, your employment may be dismissed or laid off even before
the period herein agreed upon, without any obligation on the part of the company.
6. The company reserves the right to transfer or shift from one section to another within the company premises.
7. Should you absent yourself for 5 (five) consecutive working days, without permission from Human Resource or your
immediate superior, you shall be considered to have voluntarily abandoned your employment and this employment
agreement shall be deemed terminated.
8. You shall faithfully abide by all terms and conditions of employment and all policies, rules and regulations of this
company.
9. This employment agreement shall cease to be valid on the expiry date stated above and the employee shall be
deemed separated from the company unless you receive a renewal notice.
10. In case you intend to resign from the Company, you are required to notify the Company at least thirty (30) days prior to
the effectively of your resignation, otherwise, failure on your part to do so will render liable for damages. However, it is
within the sole discretion of the Company whether or not to accept such resignation earlier than the expiration of said
period. If you agree with the above terms and conditions, please indicate your conformity by signing on the space
provided below for this purpose.
11. The employee has properly oriented with regards to the company policy/handbook.
12. The employer has the rights to terminate the employee if the employee is violated the company policy or has
unpleasant behavior that is not appropriate for the company under probationary period.
13. You certify that you have fully read and understood the contents of this agreement and committed to wholly comply.
EMPLOYMENT AGREEMENT

KNOW BY ALL MEN BY THESE PRESENTS:

That I, FRANCIS G. SALGADO, of legal age, and a resident of STO. NINO SABLAYAN OCCIDENTAL
MINDORO, after having been employed as probitionary employee of Ultimate Care
Megaventures Corporation.
That I agree to work as probitionary employee for the duration of the above- mentioned period.

That after the completion of these commitments my employment shall automatically cease
without need for further notice or advice.

That I shall do my work faithfully and sincerely, including the work that may be assigned to me
pertinent to the job called for.

That Ultimate Care Megaventures Corporation may at any time terminate my services even
before the lapse of the above-mentioned period, if and when I am found to be incompetent or
had violated the rules and regulations which are ground for dismissal, or whenever exigencies of
the service arises.

That I further understood that the terms and conditions herein prescribed by my employer shall
be binding for the duration of my employment and will not contravene rights that the company
may have against me in connection with my employment.
