Dear Amirul,
It has brought to the attention of the Management that you did not report to work on 04 & 10 June,
and 15 July 2023 at our Gigi Coffee Toppen Shopping Centre outlet without prior approval from
your superior.
Your unauthorized absence on the said dates are tantamount to gross violation of our Company’s
policy for failing to report to work without any prior approvals from superior. In view of the
above, you are hereby requested to furnish your explanation in writing within three (3) days from
the receipt of this letter as to why disciplinary action should not be taken against you. Please note
that the Management views employee’s attendance and discipline seriously, should you fail to
furnish any explanation(s) within the stipulated period, we shall assume that you have none to offer
and shall proceed to take appropriate disciplinary action against you.

deemed to have acknowledged the content of this letter, and it will be kept in your record.
