I. COURSE DESCRIPTION:

Prerequisites: Level 1 courses and Bio 203


Lecture and laboratory practice, open to all students admitted to nursing. This course will provide theoretical and laboratory experience
opportunities for students to implement assessment strategies which determine the health status of culturally diverse clients and the
maintenance of structural, personal, and social integrity throughout the lifespan.

II. COURSE OBJECTIVES:

The curriculum is designed so that students acquire and demonstrate graduate-level proficiency in general education and essential skills. The
course objectives for this course will assess each student’s proficiency in the following competencies:
1. Oral and written communication
2. Scientific and quantitative reasoning
3. Critical analysis and reasoning
4. Technological competency
5. Information literacy

Upon successful completion of this course, the student will:

1. Apply knowledge from the natural, social, and behavioral nursing sciences and humanities to conserve energy and integrities
for individuals across the lifespan.(Level II, Essential I)
2. Apply critical thinking and clinical reasoning in the delivery of nursing care for clients (Level II, Essential III)
3. Demonstrate effective communication skills to establish a nurse-client relationship during the health assessment process.
(Level II, Essential VI)
4. Identify relevant healthcare policies that influence the health assessment. (Level II, Essential III, V).
5. Identify the need to collaborate with other members of the multidisciplinary healthcare team to improve the management of
diverse clients in a variety of settings. (Level 2, Essential VI).
6. Identify health assessment data to generate a problem list that reflects the client’s ability/ inability to conserve energy and
maintain the structural, personal, and social integrity. (Level II, Essential IX).
7. Discuss professional conduct for performing an assessment. (Level II, Essential VIII).
8. Assess health promotion and disease prevention strategies to improve health outcomes. (Level II, Essential VII)
9. Discuss the impact of the energy, structural, social and personal integrity in health care. (Level II, Essential I).
10. Demonstrate the use of the nursing process and assessment using the Helene Fuld School of Nursing Conceptual Model (Levine
Conservation Model) in a variety of healthcare settings in the care of culturally diverse clients across the lifespan. (Level II, Essential

IV. CLINICAL/Laboratory Practice Objectives:

Completion of the following clinical objectives is fundamental to the student’s achievement of course objectives. Recommendation:
Students must pass clinical in order to pass the course.

1. Use appropriate techniques of therapeutic communication while assessing the client.


2. Conduct an assessment of selected clients using techniques of interviewing, inspection, palpation, percussion and auscultation.
3. Discuss the findings of the complete health history.
4.. Perform a complete health history and physical examination.
5. Document health history, assessment and health teaching.
7. Demonstrate professional conduct while performing an assessment.
8. Demonstrates effective oral and written communication.

Completion of the course objectives is fundamental to the student's achievement of course objectives. The laboratory will be
evaluated though the use of the Evaluation Tool for the course and laboratory skills checklist. Refer to the Evaluation Tool.

VI. MODES OF ASSESSMENT:

Examinations 75% (5 exams 15%)


Comprehensive Final Examination 25%
Clinical/Laboratory Practicum: Midterm and Final……………….. Pass/Fail

CLINICAL/LABATORY GRADE:

Students must achieve a clinical/laboratory rating of 2.5 on a 4.0 scale to pass the clinical/laboratory portion of the course. Thirty (30)
clinical hours will be completed as assigned. Clinical evaluation tools will be utilized for each system as well as the comprehensive
physical examination. This score signifies satisfactory achievement of clinical objectives.

Students must pass the theory and lab/clinical portion to pass the course.

VIII. EXAMINATION REVIEW:

Faculty will make every effort to return test scores to students and review examinations within one week.

X. COURSE POLICIES

Electronic Messaging

Effective May 12, 2006, Coppin State University’s electronic messaging system is an official means of communication, i.e., Microsoft
Office Outlook. Therefore, all University employees and students must use Coppin’s electronic messaging system when
communicating information electronically. This policy is intended to ensure the most efficient use of the University’s electronic
messaging system and the compliance with FERPA and other confidential/information security regulations.

Attendance – Class/clinical-Attendance is mandatory. Electronic notification and written documentation is required for all missed
classes.

1. Attendance: Attendance: Class attendance is MANDATORY. Students are expected to arrive for class ON-TIME. Class is
scheduled a posted. Anyone arriving for class after the start will have to sit on the side of the classroom nearest the door. Anyone not
arriving on time for class will be held responsible for obtaining content missed. Students are expected to be in attendance. Attendance
will be taken.

When an absence is required due to an emergency, the student must notify faculty within 24 hours. Attendance records will
be maintained for each class and forwarded to the Office of the Vice President for Academic Affairs in keeping with university
policy. The Coppin State University Attendance Policy will be enforced. Based on that policy, unexcused absences totaling
more than four (4) hours of lecture during the semester may result in a grade of “F”. An attendance roster will be circulated
at the beginning of class. Clinical attendance is mandatory.

2. Class Room Conduct: The use of mobile devices during class is prohibited. Students are not permitted to make out-going phone
calls in class. Only cellular phones with etiquette-mode for in-coming calls will be allowed. No mobile devices may be used during an
examination for any reasons including calculations. Student tape recording of classes is permitted. Refrain from side bar discussions.

3. Examinations: Exams will be given via a face-to-face and/or computerized format. Students are required to take the examination as
scheduled. The student must ensure adherence to the Student Code of Conduct at all times during the administration of assessments.
Communication with anyone other than the designated proctor is prohibited. Electronic communication, cellular communication, text
messaging, the use of alternate websites, navigating or use of alternate drives is prohibited during the testing time. After the
assessment is completed, students are not to discuss the assessment with other students. Various types of test items will be used to
include multiple choice, multiple-multiples, calculations and subjective responses.

4. Missed Exams: If a student is unable to take an exam or quiz on the scheduled date, the faculty member responsible for
administering the exam or quiz must be notified electronically at least 24 hours before the scheduled time of administration. Written
documentation must be provided regarding the reason for the missed examination. For the purposes of this course, an email
notification must be sent to Professor Setlow.

The exam must be taken by the student no later than the next scheduled class day. The decision to give a make-up exam is at the
discretion of the faculty. The format, content, and complexity of the alternate exam is also at the discretion of the faculty. This policy
also applies to the final examination for a course. Failure to follow the policy and procedure will result in a grade of zero for the
examination.

The grade achieved on the alternate make-up exam or quiz can be no higher than the highest grade achieved by classmates who
took the exam or quiz as scheduled.

5. Clinical/Laboratory Grade: Students must achieve a clinical rating of 2.5 or higher in each criteria to pass the clinical portion of the
course. This score signifies satisfactory achievement of clinical objectives. Students must pass clinical in order to pass the course.

6. Standardized Assessments: All students are required to take standardized assessments developed by a vendor identified by HFSON
faculty and administration. Failure to take these assessments will result in a grade of a zero for the assessment and a grade of“F” in the
course. For the purposes of this course, students will have one (1) opportunity to take the assessment. Refer to the modes of
assessment. The assessment dates will be scheduled by course faculty. The score achieved on the designated assessment will be
converted using the HFSON Proficiency Grading Scale or Composite Scale. Further directions for completing these assessment (s)
shall be provided by the course faculty.

Content Proficiency: A student is required to complete the designated content proficiency module, quizzes and/or test throughout the
semester. These assessments are associated with various areas of course content. A student is required to complete the designated
module or assignment BEFORE the associated unit exam within the course. Each student must present a printed verification that
demonstrates evidence of proficiency at the satisfactory level or above prior to each exam. THIS PRINTED FORM WILL BE
CONSIDERED THE ‘ADMISSION TICKET’ to take the exam. A student must present an Admission Ticket to take the assessment.

7. Web Enhanced Course Support: This course will be taught using the Tegrity Learning System, Blackboard Course Support System
and the Elsevier web site associated with the textbooks of the course. Students are encouraged to frequently visit the Blackboard and
Elsevier websites to gain full benefit of these learning environments.

are no standardized assessments for this course.

Clinical/Laboratory Attendance/Dress Code

Lab and clinical attendance are required. Failure to submit the necessary documents as listed in the modes of assessment shall result in
a clinical failure. If the student passes the theory portion of the course, but fails to submit the necessary clinical documents or
complete the required labs/evaluations or examinations this will result in a course failure and laboratory/clinical failure. The student
will be subject to failing the course which will be grounds for academic probation and/or dismissal. Additionally, the student must
receive a Comprehensive Health History and Physical Assessment Evaluation from the Preceptor. Dress Code for Clinical/laboratory
is lab coat white scrubs and white shoes.

XIII. PLAGIARISM POLICY:

Assignments (oral & written) that a student submits for any course is to be his/her original work. Any violation of this rule
constitutes plagiarism. Plagiarism includes any form of cheating on examinations, tests, quizzes and any unacknowledged
and/or undocumented use of another's writing or ideas published or unpublished, including copying or rewording
information found on the internet. A student who plagiarizes will receive a failing grade for the particular assignment.
Repurposed work will not be allowed. Students are advised to ask faculty members if repurposed work is allowed. Faculty
members shall use an electronic surveillance system such as Turnitin Work submitted with a Similarity Index of thirty (30)

percent or higher shall receive a grade of zero. Failure to use the Turnitin surveillance system shall also result in a grade of a
zero.
Additional sanctions could also be imposed. Student may receive grade of “F” for the course, referred to CSU student judiciary board,
dismissed from the School of Nursing/or the University.

NURS 217 Health Assessment


Clinical Skills Performance Assessment

Purpose: The Clinical Evaluation Tool will be used by the faculty to evaluate each student’s learning outcomes and clinical
performance. It is also used by the faculty to determine if each student has successfully achieved a passing grade in each behavioral
outcome. Clinical evaluation is a collaborative process between the student and the faculty. It is the student’s responsibility to seek
learning opportunities and to review the Clinical Evaluation Tool for the purpose of self-evaluation. It is recommended that the
student frequently review the contents of this tool. A student is required to complete an evaluation at midterm and at the completion
of the course. A student must score a minimum of 2.5 out of 4.0 in all areas.

Instruction for Use: A numerical rating for each element listed shall be evaluated. The rating scale is a range of values from “0” to
“4.0, based on the grading scale. The range of values is defined as follows: Unacceptable; Below Expected; Needs Improvement;
Meets Standard; Above Standard., Exceeds Standard, and Substantially Exceeds Standard. In the comment column, specific

explanation shall be provided for behaviors that are evaluated with a rating level for less than “Meets Standard”, “Exceeds Standard,
and Substantially Exceeds Standard.

Requirements: HIPAA, Infection Control, and Fire Safety must be completed prior to beginning clinical. This form contains a list of
skills required to be performed to proficiency. To be successful in NURS 217 Health Assessment, a student you must achieve a
proficiency level of 2.5 out of 4.0. Each student must be able to perform the assessment skills and explain the meaning and where
appropriate, the norms for the skills. The student must pass both the theory and clinical portions of this course.

Completion of the following clinical objectives is fundamental to the student’s achievement of course objectives. Students must pass
clinical in order to pass the course.

1. Use appropriate techniques of therapeutic communication while assessing the client.

2. Conduct an assessment of selected clients using techniques of interviewing, inspection, palpation, percussion and auscultation.

3. Discuss the findings of the complete health history.

4. Perform a complete health history and physical examination.

5. Document health history, assessment and health teaching.

6. Demonstrate professional conduct while performing an assessment.

7. Demonstrates effective oral and written communication.

expectations, requirements, or standards. Demonstrates inadequate knowledge
and skill. Examples include but not limited to unsafe performance, dishonest

(65-69) Below Expected: The student’s performance does not fully meet clinical 1.5
objectives, requirements or standards of practice. Demonstrates very minimal
knowledge and skill on a consistent basis and requires close supervision at all
times.

(70-74) Needs Improvement: The student’s performance does not fully meet clinical 2.0
objectives, requirements or standards of practice. Demonstrates minimal
knowledge and skill on an inconsistent basis. The student is able to perform
some skills independently, not requiring close supervision at all times.

(75) Meets Standard: The student’s meets clinical objectives, requirements, or 2.5
standards of practice. The student demonstrates adequate knowledge and skill
to perform in a competent manner.

(80-84) Above Standard: The student’s meets clinical objectives, requirements, or 3.0
standards of practice. The student demonstrates is fully sufficient and
demonstrates knowledge and skill to perform in an independent and competent
manner, requiring minimal guidance to perform behavior and objective.

(85-89) Exceeds Standards: The student’s performance consistently meets, but 3.5
regularly exceeds clinical objectives, requirements, or standards of practice.
The student is able to share knowledge with peers, functioning as an adequate
resource.

(90>) Substantially Exceeds: This rating is reserved for the highest level of clinical 4.0
performance. The performance substantially and consistently exceeds clinical
objectives, requirements, or standards of practice. The student makes an
achievement during the clinical experience. Identifies problems and solutions
that can improve nursing practice.
