AFFIDAVIT OF DESISTANCE
I, STEPHANIE O. BOTE, legal age, Filipino after having been duly sworn
to in accordance with la do hereby depose and state that;
That I am the private Complainant against the following person in the name
of CIARA MAICAH RIGDAO, residing at No BLK 3 TARVET BRGY SAN
RAFAEL TARLAC CITY for RECKLESS IMPRUDENCE RESULTING IN
DOUBLE HOMICIDE with IS No. 111-18-INV- 19G-1004, now pending before
the PROVINCILA PROSECUTOR TARLAC CITY.
That, after a thorough evaluation and recollection of the facts and
circumstances surrounding the incident, I realized that what happened arose out of
a misunderstanding and misapprehension of facts in the filing of the instant
criminal case and that the attending circumstances militate against its continued
prosecution, for the reason, I am no longer interested, and hereby desists from
further prosecuting the said criminal case against the respondent and those
objection evidence as well as the testimony

That, I am executing this affidavit of desistance to attest to the truth of the

foregoing and so that the above-mentioned I.S No- 111-18-INV- 19G-1004 could
be immediately dismissed and considered finally closed and terminated;

That, I am not forced, coerced nor gifted in executing this affidavit of

desistance but this is my own voluntary act and deed,

IN WITNESS WHEREOF, I have hereunto set my hand and affixed our
