JUDICIAL AFFIDAVIT OF PCPT AIVERLYN TEOFILO

I, PCPT AIVERLYN TEOFILO, Filipino, of legal age, residing at Brgy. San Isidro,
Tarlac City, Tarlac; after having been sworn in accordance with law depose and state that:

That Atty. Jonathan Pacifico, is the counsel who conducted and supervised my
examination as a witness at his law office in 01 Hilario St, Brgy.Ligtasan, Tarlac City, 2300
Tarlac;

That I am answering the questions asked of me based on my personal knowledge, fully

conscious that I am doing so under oath, and that I may face criminal liability for false testimony
or perjury;

The following are the Questions propounded by Atty. Jonathan Pacifico and my answers
in English language:

Question No. 1 : Please state your personal circumstances including your

current employment?
Answer : I am Police Captain Aiverlyn Teofilo, a resident of Brgy.
San Isidro, Tarlac City, and I am currently stationed at
Police Community Precinct 8, Brgy. San Sebastian, Tarlac
City, and I am a Women and Children Protection Desk
Officer.

Question No. 2 : Where were you on or about 11:00 in the evening of

February 9, 2020?
Answer : I was on duty at PCP 8, Brgy. San Sebastian, Tarlac City.

Question No. 3 : How did you come to know about this case?
Answer : On or about 11:30 in the evening of February 9, 2020,
PCPT Robert Quiambao who is also a Women and
Children Protection Desk Officer endorsed to me Mrs.
Melegrito and her daughter Sunshine.

Question No. 4 : What happened after PCPT Quiambao endorsed Mrs.

Melegrito and her daughter Sunshine?
Answer : Mrs. Melegrito narrated that she witnessed the accused
Valentino Baldovino raping her daughter Sunshine. She
grabbed Valentino and took him to our precinct.

Afterwards, I took the Sinumpaang Salaysay of Mrs.

Melegrito and Sunshine assisted them in preparing a
WCPD Complaint Form (ANNEX A). I then made requests
for Genital Examination (ANNEX B) and Physical /
Medical Examination (ANNEX C) of the victim. I also
advised PCPT Quiambao to conduct an ocular investigation
in the area where the incident occurred.

Question No. 5 : Was PCPT Quiambao able to conduct the ocular

investigation?
Answer : Yes. PCPT Quiambao together with some other officers
came back to the precinct around 12:40 in the morning of
February 10, 2020. They were able to recover a used
condom with what appeared to be a seminal fluid
discharge.

Question No. 6 : What was done with the recovered condom?

Answer : We endorsed the condom to the Evidence Custodian. I then
made a request for Semen Examination for the specimen in
the condom (ANNEX D).

Question No. 7 : Where there any more actions taken regarding the case?
Answer : Yes. Around 8:00 in the morning of February 10, 2020, I
turned over the custody of Valentino Santillan Baldovino to
the City Social Welfare and Development Officer Ronilo
Borja at the City Social Welfare Development Office at P.
Burgos St., Tarlac City. (ANNEX E & F)
