Data  Protection  Waiver  
 
In  countries  where  there  are  data  protection  requirements,  e.g.  EU  countries,  auditors  
should  obtain  the  written  permission  of  workers  to  view  personnel  files  during  the  
interviews,  via  the  use  of  data  consent  forms.    
(To  be  used  in  certain  countries  only,  auditor  must  check  where  this  is  applicable)    
 
In  order  to  administer  your  employment  your  employer  collects  and  uses  certain  personal  
information  (such  as  your  contact  details  and  pay  records).    This  information  constitutes  
your  personnel  file.  
 
Control   Union   has   been   engaged   as   an   auditor   to   review   the   working   practices   of   your  
employer   for   the   purposes   of   ensuring   compliance   with   the   Ethical   Trading   Initiative   Base  
Code.    As  part  of  this  review,  the  auditor  will  need  access  to  your  personnel  file.  
 
Please  confirm  that  you  are  happy  for  the  auditor  to  have  access  to  your  personnel  file  for  
the  purposes  of  the  review  by  signing  the  consent  form  below.  
 
No   documents   will   be   removed   from   your   file   and   no   names   or   other   information   which  
enables  individuals  to  be  identified  will  be  recorded.
