assessment Guide, 10 Application form and 3 attendance sheet
TESDA Representative: Good morning mam AC Manager, I will give you the 10 Rating
sheets, 1 Assessors Guide, 10 Candidate Guide and 10 CARS that will be needed by our
assessors.
(Assessor will enter and will show the appointment letter to the AC Manager.)
Assessor: Mam Ac Manager! Here is my appointment letter that will serve as a proof that I’
am the assigned assessor for this assessment.
Assessor to AC manager: I would like to check the completeness of tools and equipment, if
the assessment room is ventilated and illuminated and safe:
Assessor to AC Manager: I checked that the tools and equipment are complete, the
assessment room is well ventilated and illuminated and safe. So we can now start the
assessment.
AC Manager : These are the documents that you needed for this assessment here is the 10
Rating Sheets, 1 Assessors Guide, 10 CARS,10 specific instruction, 3 attendance
sheet, 10 filled-up self-assessment guide and 10 application form.
But before anything else, let me first check your attendance. As I call your name,
please come forward and present to me your admission slip. (Assessor will check and
compare whether the candidate is the same person on the attendance sheet)
I have here with me accomplished SELF ASSESSMENT GUIDE AND APPLICATION FORM.
This is an indication that you know and can perform all the units of competency
enumerated for Food and Beverage Services NC II. So before we start our assessment,
let’s have a brief orientation first.
What is Assessment?
-It is the process of collecting evidence and making judgement on whether competency has
been achieved.
-What is the purpose of the Assessment? It is for Qualification, Recruitment, and
Educational Needs.
Food and Beverage Services NC II is composed of Basic, Common and Core
Competencies. But we will be focusing on the core units of competencies, namely; Core1,
Prepare the dining room/restaurant area for service 2, Welcome guests and take food
and beverage orders 3, Promote food and beverage products 4, Provide food and
beverage services to guests 5, Provide room service and last, Receive and handle
guest concerns
Food and Beverage Services NC II has two types of assessments, the Demonstration and
Oral Interview.
Food and Beverage Services NC II is a project type of assessment which means there are
only two possible results either competent or not yet competent.
In demonstration you will perform the following activities:
1. Prepare the dining room/restaurant area for service
2. Welcome guests and take food and beverage orders
3. Promote food and beverage products
4. Provide food and beverage services to guests
5. Provide room service
6. Receive and handle guest concerns
You will be given 3 hours to perform, your task and also you have 1 hour for your oral
interview, a total of 4 hours.
For your assessment procedure, candidate’s number 1 should perform all activities while
candidate 2-10 will wait on waiting area
We also consider allowable adjustments, Is there anybody has religious practices that need
to address during the assessment, or taking any medications to be considered? Is there
anyone here have difficulties in understanding English language?
As a candidate, if you are not satisfied with the result given by me, you have a right to
appeal through writing addressed to the AC manager, immediately after the assessment.
And me as an assessor I have the right to stop the assessment if there is a danger to occur.
Upon receiving your CARS which will be given to you by the AC manager immediately after
the assessment, after 3 working days you may apply for the issuance of your NC at TESDA
Regional office just bring with you the following requirements:
a. CARS
b. passport size picture with white background and
c. php 50.00 for processing fee.
If you don’t have any questions, I will now give you the copy of specific instructions, please
read them carefully for 10 minutes, and don’t hesitate to ask questions. Okay, times up!
Candidate 1 go to practical work area and candidate 2 to 10 wait on the waiting area. (THE
ASSESSORS MUST USE THE RATING SHEET AT THIS POINT)
1. Ok candidate 1 pls. Perform 10 table napkin folds for 10 minutes. Start now…and
Stop. Prepare the dining room are for service start now… and stop. Welcome guests
and take food and beverage orders start now….and stop. Perform providing food and
beverage services to guests start now….and stop. Perform provide room service start
now….and stop and last demonstrate receiving and handling guest concerns.

provide food and beverage services to guests, provide room service and receive and handle
guest concerns you’re overall result is satisfactory I will sign (THE ASSESSOR WILL SIGN
ON THE RATING SHEET) you sign (THE CANDIDATE WILL SIGN THE RATING SHEET)
And your oral interview out of 10 question you’ve got 9 correct answer so you’ve got
satisfactory rate in your oral interview, I will sign (THE ASSESSOR WILL SIGN ON THE
RATING SHEET) you sign(THE CANDIDATE WILL SIGN THE RATING SHEET)
(POST ASSESMENT)
Ok the assessment is done. How was the assessment? How will you scale your performance
from 1-10 which 10 is the highest?
On your demonstration you have prepare the dining room/restaurant area for service
welcome guests and take food and beverage orders, promote food and beverage products
provide food and beverage services to guests, BUT I HAVE NOTICE THAT the sequence in
providing food and beverage service to guest WAS NOT PROPERLY EXECUTED WHICH IS
CRITICAL ON THE ASSESSMENT, providing room service and receive and handle guest
concerns was properly executed, . Your overall result is not satisfactory I will sign (THE
ASSESSOR WILL SIGN ON THE RATING SHEET) you sign (THE CANDIDATE WILL SIGN
THE RATING SHEET)
And your oral interview out of 10 question you’ve got 5 correct answer so you’ve got
unsatisfactory rate in your oral interview, I will sign (THE ASSESSOR WILL SIGN ON THE
RATING SHEET) you sign(THE CANDIDATE WILL SIGN THE RATING SHEET)
You may apply for re-assessment of your at any accredited assessment center in your own
convenient time. Ok
RECORD THE RESULT OF THE ASSESSMENT IN THE ATTENDANCE SHEET AND LET AC
MANAGER SIGN THE CARS AND ATTENDANCE SHEET. LET ALSO THE TESDA REP TO SIGN
THE ATTENDANCE SHEET.
ACM: okay candidate please sign this evaluation sheet for our assessor… okay here is
you’re CARS.
