周年體格檢查指引 2020-2021

1. Please fast for at least 8 hours before the blood draw. NO food or drink should be consumed
except sips of water.
抽血前請空腹最少 8 小時，不可進食及飲料，只可飲少量清水。

2. Please bring stool specimen (size similar to a pea) to the clinic for laboratory use. If
specimens transported in unsuitable containers or exposure to adverse conditions may give
inaccurate results. Members affected will be contacted for resubmission of specimens.
請帶大便樣本(如豆一般大少)供化驗用。 樣本若存放在不當容器或在不良情況下運送，
均可影響化驗結果，如有需要，我們會通知受影響會員再抽取樣本。

3. Female examinees prefer to take health check NOT during the menstruation period.
女性應避免於月經期間接受身體檢查。

4. Not suitable for confirmed / suspected pregnant women for X-ray investigation. Please
inform Medical Centre staff.
如確定或懷疑懷孕，不適宜進行Ｘ光檢驗，請通知醫務所職員。

5. Members should arrive to assigned Health Check Centre at scheduled date and time.
Present HKID Card for registration and state you are from Swire Coco-Cola.
會員請依時到達指定體檢中心，提供香港身份證登記，並註明是太古可口可樂公司員工。

6. Fill in & sign the “Health Declaration Form” specially designed for Swire Coca-Cola Limited /
Swire Coca-Cola Hong Kong Limited
請填寫及簽署太古可口可樂專用「個人健康狀況聲明書」。

6. With a written authorization from the Examinees, the Medical Examination Report would be
sent to the Human Resources Department after completing the examination.
會員需簽署授權書，同意授權醫務所將會員體檢報告交予給 Swire Coca-Cola Limited /
Swire Coca-Cola Hong Kong Limited 之人力資源部。

7. One only visit is required.

體檢只需到醫務所一次。
