4. For MAPEH, individual grades are given to each area, namely, Music, Arts, Physical Education and
Health. The quarterly grade for MAPEH is the average of the quarterly grades of the four areas.

5. The General Average is computed by dividing the sum of all final grades by the total number
of the learning areas.
Learner can be promoted or retained following the guidelines stipulated in DepEd Order 8, s. 2015.

Note: For those who will undertake remedial classes:
The equivalent of the Final Grade for remedial classes is the Remedial Class mark (RCM). The Final
Grade at the end of the school year and the Remedial Class Mark are averaged. This results in the
Recomputed Final Grade.
*In case, a student/pupil did not meet expectations in not more than two learning areas, the
Homeroom Sponsor should present a Certificate of Recomputed Final Grade duly noted by the
School Principal. This will be submitted to the division office and must be attached to Form
137 and SF 5. (Refer to DepEd No. 8 for the sample of certificate).
6. Under Remarks, write the word Passed (black) or Failed (in red ink) per learning area.
7. Write “Eligibility for Admission” to the next grade level in words.
Example: Eligible for Admission to Grade TWO.
8. Signature over Printed Name of the Homeroom Sponsor should be affixed on the designated space
and that of the Chairman of the Checking Committee on the first column across the curriculum year.
9. A non-numerical rating scale will be used to report on learner’s behavior demonstrating the Core
Values.
10. Accomplish attendance record for the grade.
11. Certificate of Transfer should be accomplished by the adviser and signed by the principal. The
date of close of school should also be indicated.
12. No Transferee pupil should be promoted or allowed to graduate unless his/ her Permanent
Records (F137 E/F137 A not F-138) from the school where he/ she was previously enrolled has
been received by the school.
