Guidelines in Accomplishing and Checking of Promotional Forms


SY 2017-2018

I: School Permanent Record


Form 137-E (Elementary)
Form 137 –A (Secondary)

1. Write the Learner’s Reference number (LRN) on the space provided.


2. Learners’ names correctly spelled with middle name and other related information should tally with
every entry in SF Forms, Form 138 and Form 137.
NOTE: Grade 1/Grade 7 Teachers should refer to the Child’s birth certificate and attach a certified
photocopy of the birth certificate, preferably from the NSO/PSA. Write the complete name of the
learner including the middle name (not just middle initial).
Name of school should be written in full. Abbreviations may be allowed if the space provided for
does not warrant it:
Example: 1. Wrong: DAPSMECS-MAIN
Right: Don Amadeo Perez, Sr. MECS-Main

3. The Final Grade for each learning area, the average of the four grading periods, should be written
in whole number.

4. For MAPEH, individual grades are given to each area, namely, Music, Arts, Physical Education and
Health. The quarterly grade for MAPEH is the average of the quarterly grades of the four areas.

5. The General Average is computed by dividing the sum of all final grades by the total number
of the learning areas.

5.1 The general average should be written in whole number.


5.2 The over-all general average should be encircled and initialed by the adviser in black.

Note: For those who will undertake remedial classes:
The equivalent of the Final Grade for remedial classes is the Remedial Class mark (RCM). The Final
Grade at the end of the school year and the Remedial Class Mark are averaged. This results in the
Recomputed Final Grade.
*In case, a student/pupil did not meet expectations in not more than two learning areas, the
Homeroom Sponsor should present a Certificate of Recomputed Final Grade duly noted by the
School Principal. This will be submitted to the division office and must be attached to Form
137 and SF 5. (Refer to DepEd No. 8 for the sample of certificate).
6. Under Remarks, write the word Passed (black) or Failed (in red ink) per learning area.
7. Write “Eligibility for Admission” to the next grade level in words.
Example: Eligible for Admission to Grade TWO.
8. Signature over Printed Name of the Homeroom Sponsor should be affixed on the designated space
and that of the Chairman of the Checking Committee on the first column across the curriculum year.
9. A non-numerical rating scale will be used to report on learner’s behavior demonstrating the Core
Values.
10. Accomplish attendance record for the grade.
11. Certificate of Transfer should be accomplished by the adviser and signed by the principal. The
date of close of school should also be indicated.
12. No Transferee pupil should be promoted or allowed to graduate unless his/ her Permanent
Records (F137 E/F137 A not F-138) from the school where he/ she was previously enrolled has
been received by the school.

II. Form 138 (Progress Report Card)


1. Name of the learner and other data should tally with form 137-E/137-A and other School Forms
NOTE: Surname first, followed by the First Name then the Middle Initial.
2. The age entered should be based from LIS records)
3. The name of school should be written in full.
4. Signature over Printed Name of the Adviser or Teacher who accomplished the form and the Principal
should be indicated.
5. Under Certificate to Transfer fill up only the portion that calls for eligibility for admission to Grade
(in words).
6. Do not accomplish the Cancellation of Eligibility to Transfer unless necessary.
7. Parents’ Signature must be accomplished up to the 3 rd Grading period.
8. Learning areas and rating entries should tally with those in Form 137-E/Form 137-A from 1st to 4th
grading and final rating.
9. For Grades 9 to 11, specialization in TLE should be indicated. (Ex. TLE (Dressmaking)
10. Enter, encircle and initial General average.
11. The portion for Final Rating of each learning area including the components of MAPEH should
be accomplished as:

12. The remarks after the final grade of each learning should be written in black ink for Passed and red
ink for Failed.

9. For Grade 11, Use prototyped School Forms. Use above guidelines in accomplishing the forms. For
more clarifications, refer to DepEd Order No. 8 s. 2015.
10. Refer to DepEd Order No. 36 s. 2016 for the Policy Guideline on Awards & Recognition
for the K to 12 BEC.
11. For the printing of the School Forms use the prescribed substance, brand and paper
size as indicated for uniformity.
