On 10th of December, 2019 I was posted to Wattayah OBS&GYNE polyclinic. This clinic is
responsible for antenatal and gynecology cases. I was interested to join this clinic and to know
what inside it and what the staff do there. Actually there were many objectives we have to
achieve them.

First the incharge nurse greet us and then she started to give us an orientation about the clinic.
She told us that the clinic is consist of reception, 6 doctors rooms , pharmacy, lab, triage room,
ultra sounds rooms, staff room, dirty utility room, instruments room, small canteen, and health
education room. Sunday, Monday, and Wednesday for antenatal. Tuesday, and Thursday for
GYNE cases.

In triage room they did vital signs, OGTT, urine sample, blood sugar monitoring, and blood test.
To diagnose if the woman have GDM the woman should be fasted for 8-12 hrs. Before the
appointment they use 82.5g of sugar and 250 ml and give the woman to drink. Then they did
blood serum test if the woman at low risk GDM they will do OGTT at 22-24 weeks of gestation.
If the woman at high risk of GDM do OGTT immediately. Common medication for antenatal
folic acid, ferrous sulphate, and heparin. For gyne the common medications are antibiotic oral,
fungal cream for vaginal infection, analgesic, and misoprostol. In antenatal we attended me and
my colleague ultra sound for patient S.M.H, 27 years old who was on her 14th week of
pregnancy, the doctor exposed her abdomen part only and started to apply the gel, but before that
she insured that her bladder is full. Then she showed us and showed the mother the structures of
the baby in ultra sound, the structures were visible in the ultra sound screen. She also showed us
the head circumference of the baby, how to detect if the fetus is male or female, fetal movement
heart of the baby, placenta, and the uterus. She told us the purpose of the pregnancy ultra sound
which are monitor the fetus growth and position, determine the baby’s sex, check for congenital
abnormalities or birth defects, monitor the levels of amniotic fluid, and measure the length of the
cervix.

I was on room 9 also for gyne cases with Dr.Sherma and sister Hafeetha. The doctor told me that
she admit the patient who have appointment and who visit the clinic for the first time. I saw 2
cases there. The first case was for patient B.S.I, 38 years old who was diagnosed with incomplete
abortion. Her last LMP was 19/8/2019 then she knew that she is pregnant. After 2 months of
pregnancy abortion occurred. Then she started to take misoprostol to remove the remaining
tissue of the fetus. She came for ultra sound to see if there is remaining tissue. There were some
tissue in endometrial cavity. The doctor did not know that if it is fibroid of remaining tissue of
the fetus. So after that the doctor decided to send the patient to Khoula hospital for blood
collection to test the following hormones, beat HCG, FSH, LH, and prolactin to know if it is
remaining tissue of fetus or something else. The second case was about uterine hematoma.
Patient W.H.F 26 years old who came for an appointment to see if there is remaining hematoma
or not. Actually in ultra sound everything was normal and no remaining hematoma in the uterus.
Moreover I saw a simple procedure which was smear test for a 58 old woman to take a sample of
tissue from her cervix to detect if there is any abnormalities. Patients instructions for this
procedure was patient should not make an appointment for her Pap test during her menstrual period.
Patient should not use vaginal medications, vaginal contraceptives, lubricants, or douches for 48
hours before her appointment. Intercourse is not recommended the night before the examination.

There is a room for minor procedures for gyne cases with special equipment for each procedure.
These are some of the equipment speculum, artery forceps, cervical dilator, and mayo scissors.
