AFFIDAVIT

I, TERESITA B. TACULOD, Filipino, of legal age, and a resident of Brgy. Cannery

Polomolok, South Cotabato, Philippines, after being duly sworn in accordance with law, depose
and state THAT:

1 I am the mother of TREXCY JEAN TACULOD;

2 In her Birth Record, it appears that the father indicated was unknown, however, his
true and biological father is HARRY ALVARA;
3 That HARRY PANGCONE ALVARA and HARRY CATAGUE ALVARA refers
to one and the same person as evidenced by the Joint Affidavit duly executed by the
witnesses who personally knew him;
4 That the reason of indicating an unknown father was that, during the time that I
registered the Birth Record of TREXCY JEAN TACULOD, his father HARRRY
ALVARA was still imprisoned, thus, with my earnest effort to register my child and
to afford me with Maternity Leave Benefit, I then decided not to include the name of
his father;
5 Further, I was still not married to HARR ALVARA at the time that I was pregnant
with TREXCY JEAN TACULOD, but I subsequently married him;
6 As such, I am executing this Affidavit for whatever legal purpose it may serve.

IN WITNESS WHEREOF, I have hereunto set my hand this 29th of July 2020 at
Polomolok, South Cotabato, Philippines.
