AFFIDAVIT OF UNDERTAKING

I, ANTHONY S. ECLEO, of legal age, Filipino citizen, and resident of Brgy. Jacquez, Municipality
of San Jose, Province of Dinagat Islands, after having been duly sworn to accordance with the law do
hereby depose and say that:

1. I have voluntarily submitted myself in the Mediation/Conciliation Conference before the Public
Attorney’s Office, San Jose, Dinagat Islands, on July 08, 2013;

2. I am the natural or biological father of minor PRINCESS ATHENA BENSURTO ECLEO who
was born outside of wedlock on December 19, 2013 at THE Rural Health Unit of Brgy. Don Ruben,
Municipality of San Jose, Province of Dinagat Islands;

3. I have agreed and undertake the following conditions in relation to the abovementioned case:

a. I will send financial support in the amount of NINE THOUSAND FIVE HUNDRED PESOS
(Php 9,500.00) per month payable in TWO (2) equal installments of FOUR THOUSAND
SEVEN HUNDRED FIFTY PESOS (Php 4,750.00) every 15th and 30th pay day of the
month;

b. Additional financial support will be given in times of medical needs of PRINCESS

ATHENA BENSURTO ECLEO;

4. I authorize the Provincial Budget Office, Provincial Treasurer, and Provincial

Accounting Office and any other necessary Provincial Office of the Province of Dinagat Islands to
make a separate check from my salary as ex-officio Board Member of the Province of Dinagat
Islands, in the aforementioned amount in favor of RACHEL ANNE V. BENSURTO, the mother of
my minor daughter PRINCESS ATHENA BENSURTO ECLEO;

5. I am executing this affidavit freely and voluntarily without anyone compelling me the contents
of which were explained and translated to me in the Cebuano-Surigaonon dialect which I speak and
understand – in order to attest to the veracity of all the foregoing.

IN WITNESS WHEREOF, we have hereunto set my hands and affix my signature this 2nd day of
August 2013 in the Municipality of San Jose, Province of Dinagat Islands, Philippines.
