JOINT – AFFIDAVIT OF UNDERTAKING

We, ANNALORD SANCHEZ and LEONILO CORVERA, both of legal age, Filipino citizen, and
residents of, Brgy. Luna, Surigao City, Surigao del Norte, and Brgy. San Juan, Surigao City, Surigao del
Norte, respectively, after having been duly sworn to accordance with the law do hereby depose and say
that:

1. We have voluntarily submitted ourselves in the Mediation/Conciliation Conference before the

Public Attorney’s Office, Surigao City / del Norte District, on March 20, 2012 under PAO
Mediation/Concilitaion Conference Control No. 2012-3-001.

2. We have agreed and undertake the following conditions in relation to the abovementioned
case:

a. To share Parental Authority over the minors, ALNORD SANCHEZ and LEONIL
SANCHEZ;

b. That I, LEONILO CORVERA, will have physical custody of the children, and I,
ANNALORD SANCHEZ, will have the right to the custody of the children everytime I wish
to have their custody;

c. That while the children are under our custody, we will provide for all the needs of the
children, including but not limited to, financial, moral, & spiritual support ;

3. Should we violate any portion of this undertaking without justifiable cause we shall be liable to
pay moral and exemplary damages in addition to the actual damages that the court may award.

4. We are executing this affidavit freely and voluntarily without anyone compelling us the
contents of which were explained and translated to us in the Cebuano-Surigaonon dialect which we
speak and understand – in order to attest to the veracity of all the foregoing.
