The information you have provided in support of your English language proficiency is insufficient.
To be considered for an offer in a Diploma/Bachelor of Nursing or Midwifery single, double, or combined
degree, the Australian Nursing and Midwifery Accreditation Council (ANMAC) requires that ALL
applicants demonstrate their English language proficiency via ONE of the three options outlined below.

What do I need to do?

Please tick and complete ALL fields in the option that applies to you. If selecting option 1, you must tick
the box, print your name, date and sign the declaration. Please upload your declaration to your QTAC
application ensuring that the scan or photo shows the entire page.

English is your primary language AND you have attended and satisfactorily completed at least six years of
primary and secondary education taught and assessed solely in English in one or more of the following
countries: Australia, Canada, New Zealand, the Republic of Ireland, South Africa, the United Kingdom,
and/ or the United States of America; at least two years of this education must have been between grades 7
and 12.

If you meet these criteria, please complete the declaration below.

- You must complete ALL fields, tick the box, print your name, date, and sign.
- Your signature must be a handwritten or an electronic signature. Typed signatures are not accepted.
- Upload your declaration to your QTAC application ensuring that the scan or photo shows the entire page.

English language proficiency tests and achieve the minimum scores: IELTS Academic, TOEFL iBT, PTE
Academic, or OET.
