1. Introduction
Hello and welcome to the Cordant OH Sugarman Health & Wellbeing Occupational
Health portal user guide.
The Sugarman Health & Wellbeing Occupational Health portal has been designed to
provide you with a user friendly technology based solution for accessing
occupational health services which must be completed as part of your pre-
employment screening with NCG.
The portal is a web based platform accessible from any internet connected device. It
enables you to:
Complete Medical Questionnaires (Pre Placement questionnaires) online
View management reports produced after you've had an appointment with an
Occupational Health Nurse Advisor or Occupational Health Physician
If there is a technical problem with the system then please send an email the
following address giving a brief description of the problem and a telephone number
we can contact you on.

2. Logging In
To ensure data security each client has a separate data base and portal address.
The URL of your Sugarman Health & Wellbeing Occupational Health portal will be
emailed to you when you are registered on the system.
You will receive two emails, one confirming you have been added to the system and
the second email will contain a link to your portal. Simply click on the link or copy it
into the address bar of your Internet browser and press enter.
Please note that emails can often arrive in your Junk/Spam
This is the log in screen you will see:

Now enter your personal email address and password.
Your username is your email address, if you are using the portal for the first time a
password will have been sent to you by email. If you do not have the email, or
cannot remember your password please email:

If you are logging in for the first time you will be asked to change your password.
Please enter a new password ensuring that it is 8-12 characters long and includes at
least one of each of the following:
Uppercase letter
Lowercase letter
Number
Symbol (i.e. @ or £)
Once logged in you will see a screen similar to this:

On the left hand side circled in red above is the navigation bar. You will use this to
find your way around the portal. Alternatively, you can click the navigation buttons
from the dashboard, circled in blue above.
3. What is a Medical Questionnaire (PPQs - Pre Placement questionnaires)?
Before you start your new job your new job with NCG we will ask you to complete a
medical questionnaire (Pre Placement Questionnaire or PPQ).
Your answers to this questionnaire will be CONFIDENTIAL to Sugarman Health &
Wellbeing Occupational Health, and will not be given to anyone else without your
permission.
The purpose of the questionnaire is to see whether you have any health problems
that could affect your ability to undertake the duties of the role you have been offered
or place you at any risk in the workplace. We may recommend adjustments or
assistance as a result of this assessment to enable you to do the job.
NCG’s aim is to promote and maintain the health of all colleagues at work.
Before health clearance is given for employment you may be contacted by the
Sugarman Health & Wellbeing Occupational Health team and may need to be seen
by an occupational health nurse advisor or physician.
3.1 Completing the Medical Questionnaire
After changing your password you will automatically be directed to the screen below;

All fields marked with a * are mandatory. For all other fields please provide as much
information as possible, however if you are unsure what to select or what
information to provide please leave these blank. Once you have completed your
personal details please click the save button.
You will now be automatically directed to the health questionnaire.
The first section is personal details. Anything you input of the previous screen will be
pulled through to this screen. Please check the information is correct and fill in any
gaps. The next section will be health declarations. Please answer all the questions
using the spaces provided to give a much extra information as possible.
When the questionnaire is complete please click the blue submit button circled red
below;

The information you provide will be assessed by an Occupational Health Nurse
Advisor. Even if you do declare a health condition it may not affect your ability to
undertake the duties of the post you have been offered. If the Nurse Advisor has no
concerns then a fitness certificate will be issued and you don't need to do anything
else.
If you declare a health condition that might affect your ability to undertake the duties
of the post you have been offered, then you will be contacted by an Occupational
Health Nurse Advisor. They will conduct an assessment and advise your employer of
any restrictions or changes to your duties that will enable you to do the job you have
been offered.

4. Management Referrals
If you have been referred to Sugarman Health & Wellbeing Occupational Health
services by your line manager, you may be asked to attend an assessment with an
Occupational Health Nurse Advisor or an Occupational Health Physician.
Following your appointment, the Occupational Health Nurse Advisor or Occupational
Health Physician will write a report containing recommendations to NCG.
These recommendations will be discussed with you at your appointment, however
you also have the opportunity to read the report.
This can now be done on the Sugarman Health& Wellbeing OH portal.
If you requested a copy of your report you will receive an email with a link to the
Sugarman Health & Wellbeing OH portal when the report is available.
Please log in as described in part two of this guide and click "my referral report" from
the navigation pane or the " my referral report " navigation button from the
dashboard, both circled in red below.
Then click the download document icon, circled in red below to download a copy of
your report.

If you attend an appointment with an Occupational Health Nurse Advisor, you will be
notified when your report is available to view. Your employer will be notified at the
same time.
If your appointment is with an Occupational Health Physician, you will be asked at
the time of the appointment if you wish to see a copy of your report before your
employer.
If you do wish to see your report first the OH portal will send an email notification that
your report is ready to view three working days before your employer.
If in that three working days you want to tell us of any concerns, you may have
regarding the reports content please click the "query report" button circled red on the
following page.
A pop up will appear, as shown below, allowing you to send us a message about
your report. Please provide a brief overview of your concerns and we will contact you
to ascertain further details.
Until we have contacted you the report will not be sent to your employer.

If you are happy with the content of the report and do not click "query report" within
those three working days, the report will automatically be sent to your employer.
5. Support
We hope this guide will allow you to use the Sugarman Health & Wellbeing OH portal
easily and efficiently, but should you encounter any difficulties using the system
Sugarman Health & Wellbeing are happy to provide support.
In the first instance please contact your Sugarman Health & Wellbeing Account
Manager who should be able to resolve most user queries.
However, if there is a technical problem with the system then please send an email
the following address giving a brief description of the problem and a telephone
number we can contact you on.
