1 Introduce yourself to the patient

2 Confirm patient ID using two patient identifiers

3 Gather articles and equipments at patient’s bedside

4 Explain the procedure to the patient, ensure patient’s privacy

5 Perform hand hygiene

6 Place the patient in supine position, lower side rails for easier access

7 Spike IV solution, prime the drip chamber of the tubing and clamp it

Increase the pressure in pressure bag by pumping until the green line is visible and
turn the stopcock off to the bag
8 Fix the transducer to the IV pole such that it is zeroed at the ‘X’ mark

9 Position the zero point of the transducer, it should be on the level with the patient
right atrium (phlebostatic axis)
10 Mark the phlebostatic axis using waterproof ink

11 Apply sterile gloves and mask

12 Connect the IV fluid to a three-way stopcock and flush the other two ports and
connect the stopcock

13 Connect CVP catheter to transducer and electrical monitor

14 Connect the CVP tubing from the client to the second side port of the stopcock

16 Turn stopcock off to client and open the port in the transducer for zeoring

17 Select zeroing in the monitor, and put the stopcock off to the port

18 Note the wave pattern of CVP and record the pressure.

19 Label the IV tubing and flush solution with the date and time they were first
used with nurse’s initials
20 Document the findings on nursing assessment sheet.
