LOAN CONTRACT

KNOW ALL MEN BY THESE PRESENTS:

This CONTRACT is made and executed by and between:

Proserpina R. Dizon of legal age, Filipino and resident of 6072 Kuliat st. Angeles City, Pampanga Hereinafter referred to
as the “LENDER”

-And-

Dolor Pamindanan of legal age, Filipino & resident at Angeles City Pampanga.

Hereinafter referred to as the “LENDEE”

WITNESSETH that

WHEREAS the LENDEE desires to loan the amount of TEN THOUSAND PESOS (P10,000.00) From the LENDER and
commits to pay back the entire amount in 4 equal payments of P3,000 commencing June 15, June 30, July 15 and July
30, Salary Deduction every payday.

CALCULATIONS:

LOANED AMOUNT: 10,000 + standard interest ( 2 mo x 10%) or P2,000.00

LOAN = P12,000.00 / 4 (Num of payouts) = P3,000.00

WHEREAS the LENDOR represents that she/he is financially able to provide the above mentioned amount.

Now therefore, for and in consideration of the foregoing premises and in mutual covenants hereinafter set forth. The
LENDOR and LENDEE hereby agree as follows.

 The entire amount P12,000.00 should be paid in 4 equal payments of 3,000.00 per cut off on the dates stated
above.
 Each payment shall be paid in cash, Salary Deductions from Payroll. It is the Lendees obligation to remit the
amount every cut off to the Lender’s Authorize person to get the amount.

PENALTIES FOR FALTERED PAYMENTS.

 Everyday of delay in paying will subject the LENDEE to a delay fee of P500 per delay payable within 5 days of
falter. This is for the first offense alone and succeeding payments go on as scheduled.
 Failure or refusal to pay the penalties and initial above mentioned delay fees will automatically be considered a
succeeding “non-payment” and shall pay the following remaining amount with additional 10% monthly.
