Genie Man-Lift Incident leads to Multiple Injuries

The Alerts are provided to all Drilling and Workover Operations Foremen and Liaisonmen so that we all learn from the
experiences of others. The knowledge of this incident could prevent similar ones from happening in the future.
Saudi Aramco policy requires the information in this Alert to be communicated to all crew members and documented on
morning reports.
What happened?
June 2014. A Welder and one Floorman were assigned to weld pad eyes to the 30” casing
conductor in the Cellar area. After completion the job they commenced lowering the Genie
Manlift used for the task. As descending the Genie Manlift boom became obstructed by a Pad
Eye which was attached to the extended or furthest end of a telescopic arm used during the
raising of the drill floor on rigging up. Suddenly, the Genie Manlift boom came free from this
obstruction, descending abruptly, and striking against a beam of the sub-base under.
Consequences:
The violent motion of the Genie Manlift resulted in the two employees injured, one
suffered two fractured wrists and a fractured jaw. The other employee suffered a
fractured in his left arm.
A Signal man shall be designated when the visibility of the Genie Lift Operator is
obstructed.
Only trained/competent/experienced personnel, having the Aramco license to
operate Genie Manlift, will be authorized to maneuver this machinery.
Only Genie Manlift having the valid six-monthly 3rd party inspection certification,
issued by an Aramco approved party, will be allowed to be operated. This subject
shall be verified in the Job Planning process
