Dropped Mast
SAFETY ALERT # 21/09

The Alerts are provided to all Drilling and Workover Operations Foremen and Liaisonmen so that we all learn from the
experiences of others. The knowledge of this incident could prevent similar ones from happening in the future.
Saudi Aramco policy requires the information in this Alert to be communicated to all crew members and documented on
morning reports.

What happened?

On August 15, 2009 at 10:00 am the operation at the


time of the incident the rig had just move to ABQQ-494.
The crews were preparing for the mast to be raised. The
crews were instructed by the TP to rig the mast for a
critical lift operation of moving the mast from a small
crown stand to a high crown stand.

When the mast was lifted to a height of approximately


20 feet the webbed slings failed and the mast fell to the
ground causing extensive damage.

Consequences:

The mast and associated equipment sustained extensive damage.

Why did it happen?

The following safety processes were found to be absent: No Game plan, no PTW process, no
JSA process, critical lift plan, PJSM and no Rigger I present for this critical lift operation.

What should be done to prevent reoccurrence?

1. Rig contractor to ensure line management staff is trained to the level of “competent
person” in their core HSE Administrative Control tools. a) PTW process, b) JSA process,
c) PJSM process, d) Aramco Lifting & Rigging standards.
2. Ensure rig contractors are following their own Safety Management System processes,
including the requisite documentation in alignment with Aramco Standards.
3. Review the Saudi Aramco Value Statement that gives safety an equal standing with the 10
core values.
4. Communicate to all personnel that safety will not be compromised for production interests.
