1 Applying knowledge of the purposes, characteristics, and uses of different

types of assessments

● Level of Development: Applying

● Date: 4/2021
● Evidence:
○ I use warm-up questions regularly as a formative assessment to check for
understanding of a previous lesson or to check for knowledge about an
upcoming lesson
○ I use embedded questions in slideshows using PearDeck to assess students’
understanding as we progress through a lesson. This helps me with the pace and
allows me to differentiate between which students are understanding and which
aren’t → example
○ I walk around the room observing and checking in with students to check
for understanding and clarify anything that I see that needs clarification.
○ I listen in to partner/group conversations to make sure that the discussions
are showing understanding
○ I have a standard summative assessment at the end of topics to gauge
the class’s understanding as a whole.

5.2 Collecting and analyzing assessment data from a variety of sources to inform
instruction.

● Level of Development: Exploring

● Date:4/2021
● Evidence:
○ When using peardeck, I read students’ answers to questions in real-time
and provide feedback instantaneously. I differentiate feedback by asking
students who show some understanding to expand or elaborate their
answers and I provide probing questions to students who show
misunderstanding or don’t understand.
○ I like to use quizizz as an assessment tool because I am able easily to see
which questions were answered incorrectly more frequently and see which
answers the students were giving, which helps me determine what I need
to reteach and if there are any specific misconceptions.
○ I use student answers in warm-up questions to see which areas I might
need to review

5.3 Reviewing data, both individually and with colleagues, to monitor student
learning

● Level of Development: Exploring

● Date: 3/2021
● Evidence:
○ As a PLC, we give 4 of the same pre/post-tests throughout the school year and
compare and discuss data and trends. → example
○ My Co-Teacher and I review formative and summative assessments
○ Within the science department, we give the same data analysis
assessment each year to track student progress throughout their 4 years.
○ With the science department, we look at state test scores and discuss
methods to improve in areas of need.
○ With the science department, we look at the D/F percentages in each
class and discuss ways to improve them.
○ I review summative assessments to determine areas that I can improve
my teaching for the following year.

5.4 Using assessment data to establish learning goals and to plan, differentiate,
and modify instruction

● Level of Development: Applying

● Date: 4/2021
● Evidence:
○ I look at preassessment data to determine the class’s prior knowledge and
then use that information to plan instruction.
○ I will change a lesson on the fly based on informal assessments if I
determine that students are not understanding
○ I will differentiate instruction through the feedback when using real-time
feedback by asking students who show some understanding to expand or
elaborate their answers and I provide probing questions to students who
show misunderstanding or don’t understand.

5.5 Involving all students in self-assessment, goal-setting, and progress

monitoring

● Level of Development: Exploring

● Date: 4/2021
● Evidence:
○ At the beginning of this semester, I had the students create academic goals and
then have been having them check on them periodically throughout the semester
→ goal template.
○ I have begun having students give themselves a rating of a 0-4 on their
understanding of specific topics along with a section that asks if they can pinpoint
specific ideas that they don’t quite understand or areas of confusion. I can then
use this to monitor progress towards goals and address specific areas of
confusion. → example

5.6 Using available technologies to assist in assessment, analysis, and

communication of student learning
● Level of Development: exploring
● Date: 4/2021
● Evidence:
○ I use google forms for many of my formal assessments because it puts the
data into easy-to-read graphs that show me how many students are
selecting specific answers (either correctly or incorrectly).
○ I use quizizz for some assessments because I like how it shows that data
after everyone has finished. It allows me to go through it on the projector
and discuss the questions that had a higher percentage of wrong answers.
○ PearDeck is a great assessment tool that uses to give real-time feedback
and makes instant adjustments to my lessons.
○ I enjoy using Edpuzzle as a way to check for understanding of specific
videos that I want the class to watch. Especially in the distance learning
format.
○ I use the google classroom question function to ask short answer
questions as warm-ups. I like how it shows all of the answers in one place
and all I have to do is take a quick scroll through to see where the
students are at.

5.7 Using assessment information to share timely and comprehensible feedback

with students and their families

● Level of Development: Emerging

● Date: 4/2021
● Evidence:
○ I provide feedback to families through our school portal.
○ I provide feedback to students based on formal and informal assessments
○ I will use blackboard (a tool to send a text, call, and email) to reach out to
parents of students who are struggling
